supraventricular tachicardia

Irena Jekova, Vessela Krasteva
The widespread application of automatic external defibrillators (AEDs) for treating out-of-hospital cardiac arrest incidents and their particular use at railway stations defines the task for 16.67 Hz power line interference elimination from the electrocardiogram (ECG). Although this problem exists only in five European countries, it has to be solved in all AEDs, which must comply with the European standard for medical equipment requirements for interchangeability and compatibility between rail systems. The elimination of the railroad interference requires a specific approach, since its frequency band overlaps with a significant part of the frequencies in the QRS spectra...

During cesarean section under regional anesthesia various anomalies of the electrocardiogram are reported such as bradycardia, supraventricular and ventricular premature beats, supraventricular paroxysmal tachicardia, S-T depression non-significant for myocardial ischemia and second degree atrioventricular block. We describe two cases of accelerated idioventricular rhythm in patients undergoing elective cesarean section with spinal anesthesia of which we have not found mention in the literature. This arrhythmia is not dangerous and does not usually evolve into ventricular fibrillation...

With the aid of intracavitary electrophysiological investigation in hospital the influence of ethmosin, an antiarrhythmic agent, on the function of atrio-ventricular conducting system was studied in 52 patients with paroxysmal supraventricular tachicardias. It was established that the drug depresses significantly the electrophysiological values of Kent's bundle, the His-Purkinje system and atrioventricular node (mainly in the retrograde conduction): and also prolongs considerably the time of the intra-atrial conduction...

Questions of preoperative diagnosis of the place of the disturbance of the normal heart excitation, spread of this excitation and distribution of places of the most frequent disposition of these foci in the heart depending on the pathology of the process are discussed. Sixty six patients with different kinds of tachicardia were examined, 40 patients were operated upon under conditions of artificial blood circulation. The examination of patients was performed by both routine and special methods (such as electrophysiological examination with the registration of 9 points of different parts of the heart simultaneously)...

In 34 patients with complex arrhythmias and conduction disturbances, programmed transesophageal electric stimulation of the left atrium was performed to determine the sinoatrial conduction time, relative refractory period of the atrioventricular conducting system, effective refractory period of the atrioventricular conducting system, that of the atria, that of an accessory tract in the Wolff-Parkinson--White syndrome, and stimulation parameters to trigger and reverse supraventricular tachicardia in order to define its mechanism...

The effects of amiodarone (2.5 mg/Kg i.v.) and of verapamil (0.1 mg/Kg i.v.) on refractory periods and on conduction of structures interested in the reciprocating circuit, as well as on the possible echo zones, were comparatively evaluated through electrophysiological test in 8 patients, four of which with W.P.W. from Kent bundle, and four with double A-V pass with reciprocating supraventricular tachicardia (RST) documented crises. None of the two drugs seems to have the theorical requirements of balanced effect to be chosen for the antiarrhythmic prophylaxis in patients with ventricular pre-excitement...
