The DRAIN STUDY evaluated a novel use of a 38-gauge Polytip® cannula for posteriorly trapped intraoperative subretinal fluid endo-drainage during pars plana vitrectomy in rhegmatogenous retinal detachment. Sixteen patients underwent this procedure, with an 81% single surgery success rate at 3 months postoperatively. Median visual acuity improved from 20/300 to 20/80. Two cases developed cystoid macular edema and epiretinal membrane. No postoperative residual subretinal fluid was observed. Internal biliary fistula is a rare but well-known complication of cholelithiasis. It is a notoriously challenging entity to diagnose and manage. Gallstones are often the causative factor in the formation of a cholecystoenteric fistula, with the most common internal biliary fistula being a cholecystoduodenal fistula followed by a cholecystocolonic fistula. Rarely, do these fistulae exist simultaneously. Here, we present an uncommon case of cholecysto-duodenocolonic fistula.

The so-called Commando procedure, initially described by David and colleagues, consists in the reconstruction of the mitro-aortic fibrous lamina by a patch that enlarges both annuli. Its use has been described to upsize the aortic and mitral annulus for double valve replacement in adolescents. A problem with the procedure for measuring the perceived midpoint of two durations is that the spacing of probe durations affects the length of the bisection point.
