BACKGROUND: Mitigating urethral injury remains a great challenge for urologists due to lack of ideal biomaterials for urethroplasty. The application of amniotic membranes (AM) over other synthetic materials make it a better potential source for urethral reconstruction. We separated the basement layer of AM to obtain denuded human amniotic scaffold (dHAS) and then inoculated primary rabbit urethral epithelial cells on the surface of dHAS to define whether this strategy minimize potential rejection and maximize the biocompatibility of human AM...

OBJECTIVE: To evaluate the validity of a single-stage dorsal inlay for recurrent peno-glandular stenosis following previous endourological or open urethroplastic surgery. Urethral glanular reconstruction included a deep dorsal incision followed by complete scar excision to create a deep groove presenting well-vascularized recipient bed ensuring appropriate graft healing. MATERIALS AND METHODS: Between April 2002 and January 2008, a total of 34 patients (mean age 51...

Cell-seeded synthetic polymer scaffolds constitute an emerging technology for urethroplastic applications. The study goal was to identify urethral proteins appropriate for cell attachment and optimize their adsorption onto two types of scaffolds: porous poly(ester urethane) with a poly(caprolactone) soft segment (PEU-PCL) and poly-(96% L/4% D)-lactic acid (P96L/4DLA). Specimens from eight men undergoing urethral reconstruction for stricture diseases were subjected to immunohistochemical analysis. Type I collagen, type IV collagen and vitronectin were detected at the interface between the epithelium and its basement membrane...

PURPOSE: Pelvic fracture urethral distraction defect is usually managed by the end to end anastomotic urethroplasty. Surgical repair of those patients with post-traumatic complex posterior urethral defects, who have undergone failed previous surgical treatments, remains one of the most challenging problems in urology. Appendix urinary diversion could be used in such cases. However, the appendix tissue is not always usable. We report our experience on management of patients with long urethral defect with history of one or more failed urethroplasties by Monti channel urinary diversion...

A 76-year-old man injured in a car accident was admitted with urinary retention and urethral bleeding. Cystoscopic findings revealed disruption of the bulbous urethra and we performed cystostomy. At sixty days after the injury, we performed endoscopic urethroplasty with a steel needle puncturing from the proximal end of the disruption to the distal end under the guidance of C-Arm fluoroscopy. We removed the urethral catheter at the 37th day after surgery and then performed urethral dilation for postoperative urethral stricture...

OBJECTIVE: To investigated the value of homologous dermal acellular matrix graft (AMG) for urethral reconstruction in man, and found an ideal replacement material to simplify the urethroplastic surgery. METHODS: 16 male patients, aged 18-46 (mean 26.5), with urethral diseases were treated with allograft of tissue engineering material AMG. The causes of urethroplasty were complex urethral stricture followed by pelvic fracture (13 cases), anterior urethral stricture (2 cases), hypospadia (1 case)...

Long tract urethral reconstruction nowadays still has no other resolution than two-stage techniques or graft and flap procedures that are neither simple nor trouble-free both for the patient and the surgeon. Tissue engineering simplifies this surgery using "porcine acellular matrix", obtained from small intestine submucosa. It is thin but strong, just ready for grafting, without rejection, because it is not immunogenic, being deprivated of cells. It serves as biological bridging of the reconstruction, promoting the generation of surrounding tissue in which it is completely transformed...

OBJECTIVE: The use of "porcine acellular matrix", obtained from small intestine submucosa, could simplify the repair of long urethral strictures, whereas single stage techniques can be carried out only by means of grafts, as buccal mucosa; or flaps, as prepucial skin. To our knowledge we report the first use of porcine intestine submucosa in urethroplastic surgery. MATERIALS AND METHODS: From May 2001 to December 2001, five urethral reconstructions were completed using "porcine acellular matrix"...

The surgical treatment of choice for bulbar-membranous urethral strictures should remove the stricture and achieve an end-to-end anastomosis between two spatulated surfaces to achieve better patency (Turner-Warwick technique [13]). When this is not possible because of recurrence, complex or longer strictures the problem is to choose between various candidate tissue types susceptible of forming a dependable neourethra. With this in view, we have used over the last twenty years a wide range of urethroplastic operating techniques using of scrotal skin [4], bladder mucosa free grafts [3], and dermo-epidermal free mesh grafts [10]...

An ideal free graft substance for urethroplasty should: 1, be well accepted at the site of urethroplasty with rapid healing; 2, not invite secondary complications, such as cysts, hair growth and calculus formation; 3, not cause scarring or recurrent stricture disease, and 4, be easy to procure and handle. Tunica vaginalis satisfies these criteria and, in this experimental study, proved to be an excellent graft material for urethroplastic procedures.

In bulbar-membranous urethral strictures, the surgical choice treatment should consist on removing the stricture and performing an end to end overlap anastomosis between two flat patent surfaces (Turner-Warwick's technique), whenever possible. When this procedure cannot be used (recurrence, complex or too long strictures), we get different types of tissue in order to create a fitting neo-urethra. To this goal, in the last twenty years we performed, a wide range of urethroplastic techniques (scrotal flap, bladder mucosa free grafts, dermo-epidermal free mesh-grafts)...
