BACKGROUND: The high genetic barrier to resistance of dolutegravir might allow for its use as maintenance monotherapy in patients with HIV. We investigated whether dolutegravir monotherapy was non-inferior to combination antiretroviral therapy (ART) for maintaining virological suppression in patients with HIV-1 infection successfully treated with combination ART. METHODS: We did this open-label, phase 2, randomised non-inferiority trial at two medical centres in the Netherlands...

Background: Children and adolescents have poorer HIV treatment outcomes than adults. We aimed to assess the effect of community-based support for caregivers of HIV-infected children and adolescents, who are key mediators to children engaging with care, on treatment outcomes. Methods: In this open-label, randomised contolled trial, we recruited children aged 6-15 years with newly-diagnosed HIV attending primary health-care clinics in Harare, Zimbabwe. Children were randomly assigned to receive decentralised primary health-care clinic-based HIV care (control group), according to national guidelines for 18 months, or decentralised care plus structured support visits by trained community health workers (intervention group) according to national guidelines for 18 months...

Even though it is well known that both the magnitude and detailed angular shape of scattering (phase function, PF), particularly in the backward angles, affect the color of the ocean, the current remote-sensing reflectance (Rrs) models typically account for the effect of its magnitude only through the backscattering coefficient (bb). Using 116 volume scattering function (VSF) measurements previously collected in three coastal waters around the U.S. and in the water of the North Atlantic Ocean, we re-examined the effect of particle PF on Rrs in four scenarios...

Three scenarios that produce colored thunderstorms are simulated. In Scenario #1, the thunderstorm's sunlit face exhibits a color gradient from white or yellow at top to red at base when the sun is near the horizon. It is simulated with a second-order scattering model as a combination of sunlight and skylight reflected from the cloud face that is attenuated and reddened by Rayleigh and Mie scattering over the long optical path near sunset that increases from cloud top to base. In Scenario #2, the base of the precipitation shaft appears luminous green-blue when surrounded by a much darker arcus cloud...

Accurate radiative transfer simulations of signals received by sensors deployed on satellite platforms for remote sensing purposes can be computationally demanding depending on channel width and the spectral variation of atmospheric and surface optical properties. Therefore, methods that can speed up such simulations are desirable. While it is common practice to use atmospheric "window" channels to minimize the influence of gaseous absorption, the impact of the underlying surface as well as clouds and aerosols has received less attention...

OBJECTIVES: Thoracic endovascular aortic repair (TEVAR) has been shown to lead to increased aortic stiffness. The aim of this study was to investigate the effect of stent graft type and stent graft length on aortic stiffness in a controlled, experimental setting. METHODS: Twenty porcine thoracic aortas were connected to a pulsatile mock loop system. Intraluminal pressure was recorded at two sites in order to measure pulse wave velocity (PWV) for each aorta: before stent graft deployment (t1); after deployment of a 100-mm long stent graft (t2); and after distal extension through deployment of a second 100-mm long stent graft (t3)...

An article in this journal in 2016 demonstrated that smoking prevalence among Arab men in Israel is greater than among their Jewish counterparts born in Israel, while the reverse is true among Arab and Jewish Israeli women. This is reflected in lung cancer mortality rates. In the U.S., smoking prevalence in the mid-1960s was 20% higher in African American men than in white men, but has since decreased in both groups, and smoking prevalence in the two groups is now nearly identical. The black-white disparity in lung cancer mortality rates has been reduced by more than half as compared to its zenith in the early 1990s...

BACKGROUND: Genesis of a cartilaginous scaffold is an obligate precursor to bone formation in heterotopic endochondral ossification (HEO). We tested the hypothesis that cartilage-derived retinoic acid-sensitive protein (CD-RAP) can serve as a plasma biomarker for the pre-osseous cartilaginous stage of HEO. Palovarotene, a retinoic acid receptor-gamma (RARγ) agonist, has been proposed as a possible treatment for fibrodysplasia ossificans progressiva (FOP) and is a potent inhibitor of HEO in mouse models...

Formaldehyde (HCHO) provides a proxy to reveal the isoprene and biogenic volatile organic compounds emission which plays important roles in atmospheric chemical process and climate change. The ground-based observation with zenith-sky DOAS is carried out in order to validate the HCHO columns from OMI. It has a good correlation of 0.71678 between the HCHO columns from two sources. Then we use the OMI HCHO columns from January 2006 to December 2015 to indicate the interannual variation and spatial distribution in Xishuangbanna...

A dual-frequency direct detection Doppler lidar is demonstrated using a superconducting nanowire single-photon detector (SNSPD) at 1.5 μm. The so-called double-edge technique is implemented by using a dual-frequency laser pulse, rather than using a double-channel Fabry-Perot interferometer. Such a modification to the reported lidars enhances the frequency stability in the system level. Using the time-division multiplexing method, only one piece of SNSPD is used in the optical receiver, making the system simplified and robust...

INTRODUCTION/OBJECTIVES: Endovascular repair of a proximal anastomotic pseudoaneurysm of an end-to-side aortobifemoral bypass (ABF) can lead to pelvic ischemia. We present a novel application of branched aortic endograft to repair such pseudoaneurysm while preserving flow into the ABF and native aortoiliac system. METHODS: A 71 year old male with history of aortoiliac occlusive disease resulting in lifestyle limiting claudication was treated with an aortobifemoral bypass in 1999...

Thoracic endovascular aortic repair (TEVAR) is a less invasive option for managing traumatic injuries of the descending aorta in polytraumatized patients. Concerns arise when treating young patients with TEVAR. A 22-year-old male was admitted to the emergency department following a high-impact road traffic collision. Whole-body computed tomography (CT) scan documented multiple injuries, including rupture of descending thoracic aorta just below the isthmus. There was no evidence of paraplegia or stroke. We decided to treat him in an endovascular fashion with a Zenith Cook (Cook Incorporated, Bloomington, IN) endograft...

BACKGROUND: Increased renal complications have been suggested with suprarenal stent grafts, but long-term analyses have been limited. Therefore, the purpose of this study was to evaluate the effect of endograft choice on perioperative and long-term outcomes. METHODS: We compared Medicare beneficiaries undergoing endovascular AAA repair from 2005-2008 with endografts with infrarenal fixation and a single docking limb (AneuRx, Excluder) to those with suprarenal fixation and two docking limbs (Zenith), or a unibody configuration (Powerlink)...

The development of protein therapeutics requires stabilization of these labile molecules during shipment and storage. Biologics, particularly monoclonal antibodies, are frequently packaged at high concentration in prefillable syringes traditionally made of glass. However, some biologics are unstable in glass due to sensitivity to silicone oil, tungsten, glue or metal ions. Syringes made from the plastic cyclic olefin polymer, Daikyo Crystal Zenith® (CZ), with a Flurotec-laminated piston have none of these issues...

The upwelling radiance attenuation coefficient (KLu) in the upper 10 m of the water column can be significantly influenced by inelastic scattering processes, and thus will vary even with homogeneous water properties. The Marine Optical BuoY (MOBY), the primary vicarious calibration site for many ocean color sensors, makes measurements of the upwelling radiance (Lu) at 1 m, 5 m, and 9 m and uses these values to determine KLu and propagate the upwelling radiance directed toward the zenith, Lu, at 1 m to and through the surface...
