We report a case of fatal anaphylactoid shock caused by a CT examination using nonionic contrast medium. A 79-year-old female patient was diagnosed with right recurrent nerve plasy. There was no known history of drug allergy or exposure to contrast medium. Approximately 50 seconds after contrast medium bolus injection began, the patient was noted to be apneic. Despite cardiopulmonary resucitation, the patient died. An autopsy demonstrated marked laryngeal edema and showed extensive mast cell infiltration.

We report a case of cardiac arrest associated with cisapride in combination with itraconazole and provide a brief review of pertinent literature. Cisapride (Propulsid; Janssen Pharmaceuticals, Titusville, NJ), a gastrointestinal prokinetic drug, has recently been reported to prolong the QT interval. Itraconazole, an oral antifungal agent, is an inhibitor of cytochrome P450 (CYP3A4) metabolism and may elevate serum drug levels of compounds metabolized by this pathway. UNLABELLED: Of the 733 patients with implantable cardioverter-defibrillators (ICDs) from 1982 to 1995 in our center, 20 died suddenly while the ICD was activated. This number included 16 men and four women with a mean age of 60 +/- 8 years and ejection fractions of 24.2% +/- 8.6%. ICDs were implanted for drug refractory ventricular tachycardia (VT) in 13 patients and for resucitated cardiac arrest in seven patients.
