Patients with left ventricular ejection fraction below 45% (mean 39+/-3.7%) were randomized either to captopril (n=33) or eprosartan after miocardial infarction (n=33) on days 3-7 of myocardial infarction. A large multicenter cohort study, the DAI study, is being performed on patients with type 2 diabetes mellitus who are followed by diabetic care units in Italy, to study the prevalence and incidence of macrovascular complications. The reference population consists of all patients visited at the participating units in the period September 1998-June 1999. We describe the case of a 35-year-old male, who presented with sudden death, secondary to anterior miocardial infarction due to embolism. Echocardiography, transthoracic and transesophageal, revealed a tumor in the anterior mitral leaflet. Tumor, histopathologically an papillary fibroelastoma, was excised and the mitral valve was replaced with a mechanical prosthetic valve. We revised the current literature on intracardiac papillary fibroelastoma.

From october '76 to march '79 at the Clinica Chirurgica I of the University of Bologna 50 renal graft have been performed with living donor in 8 cases and with cadaver donor in 42. In the living donor group there was one death for rejection and sepsis. The remaining 7 patients are all alive with normal renal function. Among the 42 patients with cadaver donor 6 died: 3 early (one for gastric hemorrage, one for necrosis of the ascending colon, and one for rejection) and 3 late (two following many rejection episodes and one for miocardial infarction)...

Obstructive lesions of the left common coronary trunk need surgery if our operability criteria: possibility of at least making a by-pass on the left descending anterior coronary branch and a not seriously compromised miocardial contractility exist. Of the 25 patients operated, there was a mortality of 8% and 3 intra-and post-operatory infarcts; two other patients died after some time (one for extracardiac causes). Other infarcts were not observed. 48% of the patients had incomplete riva-scolarization which after some time had an effect on the objective state of the patients but not on their survival...

The investigation performed has demonstrated that under a great deal of coronary, pulmonary and other organs' disorders, in the venous and lymphatic beds of the heart compensatory-adaptive and pathological changes occur. The earliest and deepest changes are noted in the microcirculatory bed. Most of morphological signs on vascular reconstruction are not specific and are observed at different kinds of clinical and experimental pathology. Changes in venous and lymphatic vessels of pathological conditions& do not only result from damage in the organ's wall or hemodynamical changes in the cavities of the cardiac vessels but they themselves can cause some disturbances in the miocardial microcirculation intensifying its hypoxia, congestive symptoms and enhancing the development of decompensation in the organ...
