Rita P Fleming-Castaldy, Glen Gillen
The occupational therapy profession has put forth a vision for evidence-based practice. Although many practitioners express a commitment to the provision of services informed by evidence, the reality that tradition still determines much of our education, certification, and practice cannot be ignored. In this article, we highlight the disconnect between the profession's aspirations and actual practices using neurophysiological models as an example. We describe actions to actualize the shift from traditional interventions to evidence-based approaches...

RANDOMIZED CONTROLLED TRIAL

OBJECTIVE: This preliminary study sought to determine whether the imagery perspective used during mental practice (MP) differentially influenced performance outcomes after stroke. METHOD: Nineteen participants with unilateral subacute stroke (9 men and 10 women, ages 28-77) were randomly allocated to one of three groups. All groups received 30-min occupational therapy sessions 2×/wk for 6 wk. Experimental groups received MP training in functional tasks using either an internal or an external perspective; the control group received relaxation imagery training...

Upper limb paresis is a major source of disability in stroke survivors, and robotic aided exercise therapy is a promising approach to enhance motor abilities. Few devices have been available to provide robotic therapy to the fingers and hand. We report an open-label pilot study of 12 individuals with chronic moderate hemiparesis after stroke who underwent a six-week training program using a hand robotic device. Participants received a total of 18 hours of robotic therapy. Improvements were found in multiple measures of motor performance, including the Upper Extremity Fugl-Meyer, the Motor Activity Log, the Manual Ability Measure-36, and the Jebsen Hand Function Test...

OBJECTIVE: We sought to determine whether mental practice is an effective intervention to improve upper-limb recovery after stroke. METHOD: We conducted a systematic review of the literature, searching electronic databases for the years 1985 to February 2009. We selected studies according to specified criteria, rated each study for level of evidence, and summarized study elements. RESULTS: Studies differed with respect to design, patient characteristics, intervention protocols, and outcome measures...

Vector diffraction theory is applied to the case of focused TEM(00) Gaussian beams passing through a spatially limiting aperture in order to investigate the propagation of these clipped focused-Gaussian beams. Beam distributions at different axial distances show that a traditional M(2) propagation model cannot be used for the propagation of clipped focus-Gaussian beams. Using Luneberg's vector diffraction theory and Fresnel approximations, an analytical model for the on-axis transverse and longitudinal electric fields and intensity distributions is presented including predictions of the maximum obtainable intensity...

Using nonparaxial vector diffraction theory derived using the Hertz vector formalism, integral expressions for the electric and magnetic field components of light within and beyond an apertured plane are obtained for an incident plane wave. For linearly polarized light incident on a circular aperture, the integrals for the field components and for the Poynting vector are numerically evaluated. By further two-dimensional integration of a Poynting vector component, the total transmission of a circular aperture is determined as a function of the aperture radius to wavelength ratio...

Hertz vector diffraction theory is applied to a focused TEM00 Gaussian light field passing through a circular aperture. The resulting theoretical vector field model reproduces plane-wave diffractive behavior for severely clipped beams, expected Gaussian beam behavior for unperturbed focused Gaussian beams as well as unique diffracted-Gaussian behavior between the two regimes. The maximum intensity obtainable and the width of the beam in the focal plane are investigated as a function of the clipping ratio between the aperture radius and the beam width in the aperture plane...

An experimental method is introduced to measure the refractive index and its temperature dependence for wafer-shaped infrared materials over a continuous temperature range. Using a combination of Michelson interferometry, Fabry-Perot interferometry, and a temperature-controlled cryostat in a laser micrometer, refractive index values and their temperature coefficients can be measured for any specific temperature within a desired temperature range. Measurements are reported for InAs and InSb for a laser wavelength of 10...

This study examined the effects of various seated trunk postures on upper extremity function. 59 adults were tested using the Jebsen Taylor Hand Function Test while in three different trunk postures. Significant mean differences between the neutral versus the flexed and laterally flexed trunk postures were noted during selected tasks. Specifically, dominant hand performance during the tasks of feeding and lifting heavy cans was significantly slower while the trunk was flexed and laterally flexed than when performed in the neutral trunk position...

The emotional impact of surviving a stroke has not received the same attention as physical aspects. This is particularly true regarding how stroke survivors cope during inpatient rehabilitation. This study examined the coping strategies used by stroke survivors undergoing inpatient rehabilitation and the relationships between demographic or clinical variables and coping behaviors. This case series examined 16 acute stroke survivors via standardized assessments and a medical records review completed during the first week of inpatient rehabilitation...

This exploratory study focused on gaining insight into the psychological experience of stroke survivors undergoing inpatient rehabilitation. Specifically it investigated whether stroke survivors undergoing inpatient rehabilitation were able to identify any positive consequence of their stroke. This phenomenon, also know as benefit finding or positive reframing, has recently received more attention in the coping literature examining reactions to severe illness. Using a case series methodology and qualitative analytic techniques, 16 stroke survivors were interviewed using standardized open-ended questions related to identification of positive consequences attributed to surviving a stroke...

We present a method to independently measure the refractive index and the thickness of materials having flat and parallel sides by using a combination of Michelson and Fabry-Perot interferometry techniques. The method has been used to determine refractive-index values in the infrared with uncertainties in the third decimal place and thicknesses accurate to within +/- 5 microm for materials at room and cryogenic temperatures.

A method to determine the absolute refractive index of materials available in the shape of flat wafers with parallel sides by using interferometric techniques is presented. With this method, nondestructive, sample-specific measurements can be made. The method is tested by using silicon, germanium and zinc selenide, and measurements for both the ordinary and extraordinary axes of ZnGeP2 for temperatures of 300 and 77 K are reported.

This case report summarizes the evaluation and treatment used to provide occupational therapy services to a man living with multiple sclerosis. Primary impairments included ataxia, paraparesis, and decreased endurance. The focus of this case study was on improving the client's ability to use powered mobility to access the community despite severe ataxia. A task-oriented approach was used as a frame of reference to guide the evaluation and intervention process. The primary goals of intervention were to control the degrees of freedom required for task participation and simultaneously increase postural stability, resulting in independent control of a power wheelchair...
