Hildegard of Bingen (1098-1179AD) is one of the most relevant figures of the Middle Ages. She wrote two medical books, Physica (Natural history) and Causae et curae (Causes and remedies). Our aim was to provide a comprehensive account of Hildegard of Bingen's conception of epilepsy, of the remedies proposed to treat it, and of the medical and physiological theories behind their use. In this invited opening Plenary Lecture at the 1st European Conference on Medical Physics, I indicated some of the roles of medical physics, and specifically medical radiation therapy physics, in the development of improved cancer care for the third millennium. It is said that you are only asked to predict the future if you are seriously old and/or will not be around long enough to know if you were correct. Hopefully, however, I will be able to contribute to this field for many years to come and many of my suggestions will be enacted...

Cancer immunotherapy aims at eliciting an immune system response against the tumor. However, it is often characterized by toxic side-effects. Limiting the tumor growth and, concurrently, avoiding the toxicity of a drug, is the problem of protocol design. We formulate this question as an optimization problem and derive an algorithm for its solution. Unlike the standard optimal control approach, the algorithm simulates impulse-like drug administrations. It relies on an exact computation of the gradient of the cost function with respect to any protocol by means of the variational equations, that can be solved in parallel with the system...
