Our approach differs from the usual global measure of cardiac efficiency by using PET/MRI to measure efficiency of small pieces of cardiac tissue whose limiting size is equal to the spatial resolution of the PET scanner. C-acetate uptake could be increased in physiologic or inflammatory conditions without evidence of cancer. We report a hepatocellular carcinoma patient with sorafenib-induced metabolic acidosis, who showed increased hepatic uptake of C-acetate. C-acetate PET/CT might be a potentially useful surrogate marker to monitor these adverse effects via the changes in C-acetate uptake patterns from before to after drug withdrawal.

Gastrointestinal tract is rarely a metastatic site of hepatocellular carcinoma (HCC). We present a 43-year-old woman with small bowel metastasis from HCC in which metastasis was detected using F-FDG PET/CT. However, the site of metastasis showed no significant uptake on C-acetate PET/CT. C-acetate PET/CT has a limited value in HCC patients with suspected extrahepatic metastasis, compared with F-FDG PET/CT.
