Verruca plana, also known as flat wart, is a common viral skin infection by human papillomavirus types 3, 10, 28 and 41. It's mostly prevalent on the facial area of children and young adults, becoming a heavy cosmetic burden on these patients. In this study, we report 6 cases of verruca plana predominantly affecting the visage, which are treated with 10% 5-aminolevulinic acid-based photodynamic therapy (ALA-PDT). Epidermodysplasia verruciformis is a rare genodermatosis associated with mutations in the EVER1/TMC6 and EVER2/TMC8 genes. The inheritance is considered to be autosomal recessive, but reports suggesting an autosomal dominant inheritance indicate disease genetic heterogeneity. Its onset occurs in early childhood and presents as a combination of pityriasis versicolor-like, flat wart-like and seborrheic keratosis-like lesions, with a potential for malignant transformation, mainly squamous cell carcinoma.

We present a 44-year-old man with multiple flat papules over a tattoo, diagnosed with acquired verruciform epidermodysplasia (EV). The lesions completely disappeared after 3 sessions of photodynamic therapy (PDT) with topical methyl aminolevulinate. PDT could be considered a treatment of choice in human papillomavirus lesions located over tattoos since it resolves the lesions while preserving the integrity of the design.

A healthy 21-year-old woman presented with multiple, 1-4 mm, skin-coloured papules following the ink pattern of a feather-shaped tattoo; this was diagnosed as a human-papilloma-virus induced cutaneous verrucae plana (flat wart).
