(1) Background: Participation in ultra-endurance sports, particularly ultra-running, has increased over the previous three decades. These are accompanied by high energetic demands, which may be further exacerbated by extreme environmental conditions. Preparation is long-term, comprising of sufficient exercise management, supportive dietary habits, and nutritional intakes for optimal adaptations. Gastrointestinal symptoms are often cited as causing underperformance and incompletion of events. Though the majority do not pose serious long-term health risks, they may still arise...

Intense, long exercise can increase oxidative stress, leading to higher levels of inflammatory mediators and muscle damage. At the same time, fatigue has been suggested as one of the factors giving rise to delayed-onset muscle soreness (DOMS). The aim of this study was to investigate the efficacy of a specific electrical stimulation (ES) treatment (without elicited muscular contraction) on two different scenarios: in the laboratory on eleven healthy volunteers (56.45 ± 4.87 years) after upper limbs eccentric exercise (Study 1) and in the field on fourteen ultra-endurance athletes (age 47...

Carbohydrate (CHO) intake recommendations for events lasting longer than 3h indicate that athletes should ingest up to 90g.h.-1 of multiple transportable carbohydrates (MTC). We examined the dietary intake of amateur (males: n =11, females: n =7) ultra-endurance runners (mean age and mass 41.5±5.1years and 75.8±11.7kg) prior to, and during a 24-h ultra-endurance event. Heart rate and interstitial glucose concentration (indwelling sensor) were also tracked throughout the event. Pre-race diet (each 24 over 48h) was recorded via weighed intake and included the pre-race meal (1-4h pre-race)...

In recent years, increasing numbers of women have participated in extremely long races. In adult males, there is a clear association between physiological levels of endogenous sex hormones and physical performance. However, the influence of plasmatic sex hormones and the effects of different types of hormonal contraception (HC) on the modulation of physical performance in adult females remain to be fully clarified. Eighteen female ultra-endurance athletes were recruited to participate in the study. Different variables were studied, including hematological parameters, body mass index, and body composition...

REVIEW
Volker Scheer, Brian J Krabak
Ultra-endurance running (UER) has seen an important increase in participation over the last few decades. Long hours of UER can lead to excessive stress on the body, resulting in musculoskeletal injuries (MSKI). UER is not a uniform sport and events can differ considerably in distance (over 42.195 km), time (e.g., events over 6 h) and multi-day or multi-stage events on various surfaces (e.g., track, on-road, off-road). The aims of this scoping review were therefore: (1) to examine the current evidence of MSKI, providing a synthesis of the most common MSKI by anatomical region and specific diagnosis; (2) categorize MSKI by type of UER activity (competition: time-limited; multi-stage; continuous UER events and training); (3) describe knowledge gaps in the literature and provide advice on potential further research...

Minimal research has examined psychological processes underpinning ultra-marathon runners' performance. This study examined the relationships between mental toughness and self-efficacy with performance in an elite sample of ultra-marathon runners competing in the 2019 Hawaiian Ultra Running Team's Trail 100-mile endurance run (HURT100). The Mental Toughness Questionnaire (SMTQ) and the Endurance Sport Self-Efficacy Scale (ESSES) were completed by 56 elite ultra-marathon runners in the HURT100 (38 males, 18 females; Mage = 38...

BACKGROUND: Ultra running is popular among youth athletes (< 19 years of age), with an exponential increase in participation over the last years. If running extreme distances during such a young age has implications in adulthood is currently unknown. METHODS: We performed a retrospective survey study investigating adults that participated in ultra endurance running as youth athletes, describing running history, health and social data. Ultra-running comprises running events longer than a marathon (>42.2 km). The prolonged duration of ultra-running leads to decrements in most or all physiological parameters and considerable energy expenditure (EE) and energy deficits. SG, 47 years, 162.5 cm, 49 kg, VO2max 4 mL/kg/min-1 /2.37 L/min-1 , ran continuously for 7 days on a treadmill in 3 h blocks followed by 30 min breaks and slept from 1-5 a.m. Heart rate (HR) oxygen uptake (VO2 ), rating of perceived exertion, weight, blood lactate (mmol·L-1 ), haemoglobin (g·dL), haematocrit (%) and glucose (mmol·L-1 ), and nutrition and hydration were recorded...

We present a case of severe, acute kidney injury, rhabdomyolysis and dehydration in a 49-year-old, competitive trail runner, after a 110-km trail race in mountainous terrain. Six days after the event, he presented to the hospital with fatigue, weight gain and oedema. Biochemically the diagnosis of severe, acute kidney injury was made, with increased serum creatinine levels of 13.4 mg/dL (normal range 0.67-1.17 mg/dL). He remained hospitalised for two weeks, and improved with conservative measures, without the need for renal replacement therapy...

This study aims to identify the different types of social interactions that occur during a multi-day, ultra-race and to understand how these interactions influence ultra-runners' race experience. METHODS: Athletes competing in Britain's "Spine Race" were recruited for the current study...

Ultra-endurance running provides numerous physiological, psychological, and nutritional challenges to the athlete and supporting practitioners. We describe the changes in physiological status, psychological condition, and nutritional intake over the course of two 100-mile running races, with differing outcomes: non-completion and completion. Athlete perception of pain, freshness, and motivation differed between events, independent of rating of perceived exertion. Our data suggest that the integration of multiple sensations (freshness, motivation, hunger, pain, and thirst) produce performance...

During extreme exercise, the body's ability to utilize energy efficiently can affect an athlete's performance. Elevated hormonal concentrations can have many benefits in regards to an athlete's overall performance during a competition. The sex difference in marathon running is increased with lower participation of women than men, but whether this occurs for ultramarathon running is not known. The study purpose was to determine whether the sex difference in performance widens among lower-placed runners and the association between the sex difference in running speed and participation rates. The top-10 ultramarathon running times, age at performance date, and the number of men and women finishers were analyzed from 20 races (45-160 km) in the US Track and Field Ultra Running Grand Prix...

We demonstrate experimentally a novel type of coherent low cost Gigabit-to-the-User Ultra-Dense-Wavelength Division Multiplexing (UD-WDM) PON, featuring 6.25 GHz channel spacing and long reach. Polarization-independent coherent detection is achieved by exploiting a novel scheme which requires only a 3 × 3 coupler, three photodiodes, basic analogue processing and a common DFB as local oscillator (LO). This avoids the conventional polarization diversity approach. The DFB LO is free running, i.e. not locked in frequency, and is tuned to detect any of the eight channels by simply changing its temperature in a range of 2° C...

Docetaxel is a semi-synthetic taxane with cytotoxic anti-neoplastic activity and, currently used as anticancer agent in several types of cancer. Docetaxel is highly bound to plasma proteins, and this significantly determines its clearance and activity. Therefore, measurement of free docetaxel in plasma is pharmacologically important when pharmacokinetics is investigated. We developed and validated chromatographic methods by ultra-performance liquid chromatography-tandem mass spectrometry to measure total and free docetaxel concentration in human plasma...
