INTRODUCTION: Very few studies exist on the use of diltiazem in the prehospital setting. Some practitioners believe this medication is prone to causing hypotension in this setting. Our goals were to determine whether the prehospital administration of diltiazem induced hypotension and to evaluate the efficacy of the drug. METHODS: Our two-tiered system is located in a suburban region of New Jersey with advanced life support (ALS) care provided by fly-car units. The ALS units do not transport patients, and all of them are hospital based...

The emergeny physician summoned to a sudden, unexpected death is confronted by various uncommon problems such as the certification of death on the "frontline" without destroying important evidence, the estimation of time since death, deciding whether the death must be reported to the authorities as well as taking care of mourning relatives. To all this questions the authors offer answers and practical advices.

OBJECTIVE: To study changes in health services consumption following substantial reduction in the availability of local emergency services in a small municipality population. METHOD: A dynamic cohort (21,000 residents of Viborg County, Denmark, of which 2,300 from Morsø municipality) was followed, 1997-2003. Data were extracted from administrative registries including information on individual use of emergency services and other hospital care, contact with GPs and socioeconomic background...

UNLABELLED: Regarding angiotensin-converting enzyme inhibitors (ACEI) poisoning, only few data are available in the last decade literature. In the previous couple of years especially isolated case reports were published. MATERIAL AND METHOD: We analyzed retrospectively all the patients with acute ACEI poisoning admitted in Iaşi Internal Medicine and Toxicology Clinic between 2004 and 2009. RESULTS: 17 cases of poisoning were recorded (enalapril-9 cases, captopril-3 cases, perindopril-3 cases, lisinopril-2 cases)...

The last few years have withnessed the significant increase of febrile neutropenias caused by diphteroids. In fact, with the development of bacterologically protected units, saprophytic bacteria--including diphteroids 0 became increasingly important in the aethiology of febrile neutropenias. Over the period from April 1986 to December 1992, in the bacteriologically protected unit of the Institute, 14 diphteroids were isolated from blood cultures in 14 patients with neoplastic disorders (12 males and 2 females), aged 24-70 years (median 44 years) treated with aggressive chemotherapy...

The Cuban national Integrated Medical Emergeny System or "Sistema Integrado de Urgencias Medicas" (SIUM) was formed in 1997. In 1998, the SIUM began an active out-of-hospital thrombolysis program using Heberkinasa, the only streptokinase obtained through recombinant DNA techniques, produced by the Cuban Center for Genetic Engineering and Biotechnology. An active community training program has also been implemented, standardizing training for the almost 20,000 members of the national emergency medical services...

A total of 18,876 urine samples from patients of clinically suspected UTI including asymptomatic pregnant females were analysed, during the year 1997 & 1998. Culture positivity rate was found to be 37%. The incidence of Proteus was on the rise during the year 1998 especially in hospitalized patients. The antibiotic sensitivity pattern of uropathogens was also analysed to check the emergence of drug resistance. This study has revealed that withdrawal of a particular antibiotic due to high level of resistance observed during previous years, has led to re-emergence of antibiotic sensitive mutants...
