OBJECTIVE: To investigate the role of diffusion-weighted magnetic resonance imaging (DWMRI) in the diagnosis of abscess-complicating fistula-in-ano. METHODS: This retrospective study was approved by our Institutional Review Board and informed consent was waived. MRI examinations, including fat-suppressed T2-weighted turbo spin-echo (T2-TSE) MRI and DWMRI, of 24 patients with a fistula-in-ano, were reviewed by two independent readers for the presence and number of visible fistulas, conspicuity and apparent diffusion coefficient (ADC) measurement of suspected fistula tracks and pelvic collections...

AIM: To assess agreement between different forms of T2 weighted imaging (T2WI), and post-contrast T1WI in the depiction of fistula tracts, inflammation, and internal openings with that of a reference test. METHODS: Thirty-nine consecutive prospective cases were enrolled. The following sequences were used for T2WI: 2D turbo-spin-echo (2D T2 TSE); 3D T2 TSE; short tau inversion recovery (STIR); 2D T2 TSE with fat saturation performed in all patients. T1WI were either a 3D T1-weighted prepared gradient echo sequence with fat saturation or a 2D T1 fat saturation [Spectral presaturation with inversion (SPIR)]...

The author makes an assessment of results of treatment of recurrent pararectal fistulas of extrasphincteric pararectal fistulas with liquidation of internal opening of the fistula using a combination of bringing down the mucous-muscular coat of the rectum and transfer of the anocutaneous island flap on the feeding pedicle from the subcutaneous fat. After operation the superficial inflammation of the wound of the perianal area was noted in 9 patients, primary wound healing at the place of connection of the cutaneous and mucous-muscular flap was observed in 28 patients...

Epidemiological features of proctological disease are not well known because available studies have provided variable conclusions. Risk factors frequently mentioned include heredity, high socioeconomic level, obesity, smoking, a diet rich in fats, alcohol, spices and pepper as well low fluid intake have all been implicated. While treating patients with various ano-rectal pathologies, apart from definitive treatment, various additional or supportive therapies are being advised both by the family physicians and surgeons...

PURPOSE: To retrospectively determine the additional value of diffusion-weighted magnetic resonance imaging (MRI) to T2-weighted imaging in the evaluation of anal fistulae in comparison with gadolinium (Gd)-enhanced imaging. MATERIALS AND METHODS: Thirteen patients (mean age, 35.2 years) with 20 anal fistulae were included. The protocol consisted of fat-suppressed T2-weighted fast spin-echo, diffusion-weighted single-shot echo-planar (b factors 0 and 800 s/mm(2)), and fat-suppressed Gd-enhanced T1-weighted gradient echo sequences...

Perianal fistula is an abnormal communication between the anal canal and perianal skin. The majority of perianal fistulous disease results from either cryptoglandular inflammation or Crohn's disease. These groups differ in pathophysiology, prognosis, and strategies for imaging and treatment. Endoanal ultrasound and magnetic resonance imaging represent current imaging strategies for evaluating perianal fistulas and may be used alone or in combination. The use of three-dimensional technique and peroxide fistulography optimize the ultrasound evaluation of perianal fistula...

OBJECTIVE: This study was carried out to evaluate the role of magnetic resonance imaging (MRI) in preoperative assessment of fistula-in-ano. SUBJECTS AND METHODS: Twenty-six patients (21 male and 5 female, age 19-65 years) were prospectively studied from July 1999 to December 2001 using a 1.0-tesla superconducting magnet. T(1)-weighted fast spin echo (T1W FSE) images before and after gadolinium injection and fat suppressed T2-weighted fast spin echo (T2W FSE) images were obtained in transverse and coronal planes...

A good digital examination is usually sufficient for the diagnosis and the treatment planning of anal fistulae. Cross-sectional imaging techniques, however, can accurately identify deep abscesses and characterize complex fistulae. MRI is well suited for this examination, with almost no motion artifact, excellent contrast between muscles and fatty spaces, and multiplanar acquisition. A fistula starts from an internal opening in the digestive tube and can end in an abscess cavity or open at the skin at an external opening...

PURPOSE: To assess the value of MRI performed with phased-array coil in the diagnosis and preoperative staging of perianal and anal fistulas. MATERIALS AND METHODS: 20 patients (13 with Crohn's disease) with clinical evidence or suspicion of anal fistulas underwent pelvic MRI (1.5 T) performed with phased-array coil. Images were obtained in the axial and coronal planes using TSE T2-weighted high resolution sequences with and without fat suppression, T2-weighted HASTE and T1-weighted FLASH sequences, with and without fat suppression, before and after gadolinium enhancement...

Major technical advances in MR imaging have led to its wider use in the evaluation of abdominal disease. The principle new pulse sequence is the RARE sequence for T2-weighted imaging. Multishot and breath-hold single-shot RARE techniques are now widely used, and both have performed as well as conventional spin-echo imaging with far shorter acquisition times. The most notable improvements have been in the detection and characterization of hepatic lesions. Two liver-specific contrast agents received FDA approval during 1997: SPIO particles or ferumoxide and mangafodipir trisodium, a hepatocyte-specific agent...

Patients with clinically suspected anorectal sepsis were studied using MRI in order to determine if T2 weighted sequences with fat suppression conveyed any additional benefit over conventional short tau inversion recovery (STIR) sequences. 23 consecutive patients (16 male) undergoing MRI for suspected perianal sepsis were studied prospectively using a 1.0 T whole body system and body coil. Axial and coronal T1 weighted turbo spin echo sequences were obtained, followed by STIR and T2 weighted spectral fat saturation inversion recovery (SPIR) sequences...

INTRODUCTION: Anal fistulae are pathological conditions observed in infections of the Hermann and Defosses' glands or related to Crohn's diseases. The success and the lack of complication of surgical treatment depends on how accurately the tracks are assessed. The contribution of MRI in anal fistulas is now well established but the imaging appearance is not well discribed. The purpose of this work was to discribe the different patterns of lesions. MATERIAL AND METHODS: Eighteen patients with anal fistulas were examined with MRI before operation (mean delay: 14 days...
