Various therapeutic methods are available to treat patients with intertrochantric fracture, which is usually caused by falling down. This complication is usually observed among the elderly, particularly old women. The choice of the proper therapeutic method depends on many factors including patient's condition, type of fracture, and the amount of movement. Hip arthroplasty is one of these therapeutic methods, which have certain advantages and disadvantages. Immediate resumption of walking with the ability of weight toleration and absence of complications such as aseptic necrosis are some advantages of this method...

Hip dislocation occurs when the femur does not break and the force is transmitted towards the hip joint leading to a posterior dislocation of the femoral head with or without posterior acetabular fracture. We present the case of a 26 year old patient involved in motor vehicle accident. Clinical and imaging (radiography, CT) examination revealed an intertrochanteric fracture associated with ipsilateral posterior hip dislocation and posterior acetabular wall fracture simultaneously with epsilateraal tibial and fibular fractures...

BACKGROUND: Traumatic femoral head fracture without dislocation with ipsilateral intertrochanteric fracture is an extremely rare injury and has not been reported in the literature. CASE REPORT: We reported a case of simultaneous ipsilateral femoral head and intertrochantric fracture without dislocation of the hip in a 74-year-old woman. The patient presented with a history of road traffic accident. Radiographs and computerised tomography scans revealed a right intertrochantric fracture and femoral head fracture without dislocation of the hip...

Pertrochantric femoral fractures are one of the most common fracture in old patients with variety of complications. However fixing Pertrochantric femoral fractures properly is clinically challenging. We report the outcome of pertrochantric femoral fractures treated with Proximal Femoral Locking Compression Plate (PFLCP) using Minimal Invasive Percutaneous Plate Osteosynthesis (MIPPO) techniques which were evaluated clinically by Harris hip score and radiologically for a union at fracture site and implant related complication...

Traumatic hip dislocations are common in high-energy motor vehicle accidents. We present a case of a 43-year old man who sustained posterior hip dislocation with posterior wall acetabular fracture and ipsilateral intertrochantric fracture following a motorcycle accident. Urgent open reduction and internal fixation of the hip fracture-dislocation and fixation of intertrochantric fracture with a dynamic hip screw were done. To our knowledge, such an injury has been rarely reported in the literature. Possible mechanisms of injury and operative procedures are discussed...

We undertook a prospective open, non-randomised study to determine the efficacy of out-patient treatment of postoperative superficial wound infection, in patients with orthopaedic implants using single daily IM injection of ceftriaxone and to examine the cost effectiveness when compared with inpatient management. The study comprised of 25 patients who have had primary total hip or knee replacements or Dynamic Hip Screw fixation for an intertrochantric fracture. The superficial wound infections were categorised into mild, moderate and severe according to the classification described in this article and the length of administration of antibiotic varied according to the severity of infection...

In developing countries, due to limited availability of modern anesthesia and overcrowding of the hospitals with patients who need surgery, high-risk patients with "intertrochanteric" fractures remain unsuita ble for open reduction and internal fixation. The aim of this study was to analyze the results of external fixation of "intertrochanteric" fractures in high-risk geriatric patients in a developing country. The results of 62 ambulatory high-risk geriatric patients with a mean age of 70 years (range 58-90 years) with "intertrochanteric" fractures, in whom external fixation was performed, are reported...

BACKGROUND: The management of unstable osteoporotic intertrochantric fractures in elderly is challenging because of difficult anatomical reduction, poor bone quality, and sometimes a need to protect the fracture from stresses of weight bearing. Internal fixation in these cases usually involves prolonged bed rest or limited ambulation, to prevent implant failure secondary to osteoporosis. This might result in higher chances of complications like pulmonary embolism, deep vein thrombosis, pneumonia, and decubitus ulcer...

Between 1933-1995, 16 patients underwent conversion-total hip replacement after internal fixation of an intertrochanteric fracture failed. Clinical results were unsatisfactory compared to primary total hip replacement. There was a high incidence of intra- and postoperative complications, including femoral fracture, wound infection, and aseptic loosening. This study should increase the surgeon's awareness of the difficulties encountered in conversion of failed intertrochanteric fractures to total hip replacement...

A study of 500, cases with follow up period of 1 to 10 years were treated by conservative and operative means. The result of conservative treatment in type III and IV fractures were comparatively favourable with result of operative treatment in same grade of fractures. It can be concluded that if an anatomical reduction was achieved by proportioned vertical and horizontal traction forces and the reduction was monitored by periodic skiagram till sound clinical and reasonable radiological union, good functional end result could be obtained...
