BACKGROUND: Postpartum venous thromboembolism (VTE) incidence differs by race and ethnicity in the United States. However, it is unclear whether eligibility criteria for postpartum VTE prophylaxis mirrors this disparity. METHODS: We analyzed the National Inpatient Sample from October 2015 through December 2019, using diagnosis and procedure codes to identify postpartum individuals and their VTE risk factors. We compared proportion of delivery hospitalizations meeting eligibility for thromboprophylaxis stratified by race/ethnicity, according to American College of Gynecology and Obstetrics (ACOG), American College of Chest Physicians (ACCP), Royal College of Obstetricians and Gynecologists (RCOG) and American Society for Hematology (ASH) guidelines...

OBJECTIVES: Hypertensive disorders of pregnancy (HDP) exert a heavy mortality burden in low- to middle-income countries (LMIC). ACOG revised HDP diagnostic guidelines to improve identifying pregnancies at greatest risk but whether they are used in LMIC is unknown. STUDY DESIGN: We held a workshop to review ACOG guidelines in La Paz, Bolivia (BO) and then reviewed prenatal, labor and delivery records for all HDP diagnoses and twice as many controls at its three largest delivery sites during the year before and the nine months after a workshop (n = 1376 cases, 2851 controls during the two periods)...

BACKGROUND: Intracervical Foley balloons (FB) are commonly used for cervical ripening, but there has been a historical concern regarding an increased risk of clinical chorioamnionitis with FB use in GBS positive patients. OBJECTIVE: We aimed to determine whether use of intracervical FB in GBS positive patients is associated with an increased risk of clinical chorioamnionitis. STUDY DESIGN: This was a secondary analysis of a randomized controlled trial (FOR MOMI) that compared cervical ripening agents within a standardized labor protocol...

Background: Postpartum contraceptive use can help prevent short-interval pregnancies, which have been associated with adverse neonatal and maternal health outcomes. Many contraceptive methods are safe for postpartum use, but patients and providers may be confused as to what impact hormonal contraception has on lactation. We performed a scoping review of the most recent U.S.-based guidelines regarding hormonal contraception on lactation to provide synthesis and recommendations to aid providers in counseling their patients...

BACKGROUND: Guidelines promote stratification for the risk for postpartum hemorrhage among parturients, although the evidence for the associated differential morbidity among the groups remains inconsistent among published reports. OBJECTIVE: Using the California Maternal Quality Care Collaborative schema modified by the American College of Obstetrics and Gynecology, we compared the composite maternal hemorrhagic outcome and the composite neonatal adverse outcome among singletons who were categorized after delivery by the researchers as low-, medium-, or high-risk for postpartum hemorrhage...

Obstetrician-gynecologists (ob-gyns) are essential to providing high-quality health care, and this duty remains unchanged during pandemics. This Committee Statement discusses ethics related to the provision of obstetric and gynecologic care during a pandemic caused by a highly transmissible pathogen. As health care guidelines related to pandemics are created by institutions, ob-gyns have a responsibility to advocate for obstetric and gynecologic health priorities. Additionally, many clinical practice decisions made to reduce the spread of the infectious agent and maximize physicians' ability to care for those who need help will have ramifications on patient satisfaction, the patient-physician relationship, and equity in health outcomes...

BACKGROUND: Risk-appropriate care improves outcomes by ensuring birthing people and infants receive care at a facility prepared to meet their needs. Perinatal regionalization has particular importance in rural areas where pregnant people might not live in a community with a birthing facility or specialty care. Limited research focuses on operationalizing risk-appropriate care in rural and remote settings. Through the implementation of the Centers for Disease Control and Prevention (CDC) Levels of Care Assessment Tool (LOCATe), this study assessed the system of risk-appropriate perinatal care in Montana...

Twin gestations are associated with increased risk of pregnancy complications. However, high-quality evidence regarding the management of twin pregnancies is limited, often resulting in inconsistencies in the recommendations of various national and international professional societies. In addition, some recommendations related to the management of twin gestations are often missing from the clinical guidelines dedicated to twin pregnancies and are instead included in the practice guidelines on specific pregnancy complications (eg, preterm birth) of the same professional society...

BACKGROUND: Early pregnancy loss is a common medical problem, and recommended treatments overlap with those used for induced abortion. The American College of Obstetricians and Gynecologists (ACOG) recommends incorporation of clinical and patient factors when applying conservative published imaging guidelines to determine timing of intervention for early pregnancy loss. However, in places where abortion is heavily regulated, clinicians managing early pregnancy loss may cautiously rely on the strictest criteria to differentiate early pregnancy loss from potentially-viable pregnancy...

PURPOSE OF REVIEW: Chronic hypertension affects up to 10% of pregnancies in the United States and the incidence of hypertensive diseases of pregnancy has more than doubled in the past decade, affecting minority women at disproportionate rates. Recent data show potential benefit by lowering the threshold of blood pressure treatment for pregnant women to >140/90 mmHg. RECENT FINDINGS: In April 2022, the results of the Chronic Hypertension and Pregnancy (CHAP) trial was published and demonstrated that lower thresholds (>140/90 vs...
