Familial dysautonomia (FD) is a genetic disease of the autonomous and sensory nervous systems. Severe gastro-oesophageal reflux is common and one of the major complications. We present an unusual case of achalasia presenting with dyspnoea and persistent cough. These symptoms persisted for months, leading to the patient undergoing a chest X-ray by her general practitioner which showed right basal consolidation and a density extending along the right mediastinum. CT scan was done which revealed megaoesophagus with a diameter of 7 cm causing tracheal compression, as well as right basal consolidation, consistent with aspiration. Oesophageal dilatation following laparoscopic-assisted gastric banding (LAGB) is a well-described complication reported in up to 80% of LAGB patients [ 1, 2]. Severe oesophageal dilatation in addition to aperistalsis is known as megaoesophagus. Symptomatic compression cardiac compression from oesophageal dilatation as a complication of LAGB is an extremely rare complication and is presented in this case report.

Achalasia is usually treated endoscopically by dilations, injection of botulinum toxin or the most recent POEM treatment, or surgically by Heller myotomy associated with fundoplication. Esophagectomy is reserved in terminal cases (megaesophagus with dysmotility or malignancy). We present a case of achalasia that, 10 years after diagnosis, without follow-up, presented severe respiratory failure secondary to compression of the airway by the megaesophagus.
