BACKGROUND/OBJECTIVE: Target vessels related complications are one of the most important 'Achille's heel' of complex thoracoabdominal endovascular procedures. The aim of this report is to describe a case of spontaneous bridging stent-graft (BSG) delayed expansion in a patient treated for type III mega-aortic syndrome, associated with aberrant right subclavian artery and independent origin of the two common carotid arteries. METHODS: The patient underwent different surgical procedures (ascending aorta replacement with carotid arteries debranching, bilateral carotid-subclavian bypass with subclavian origins embolization and TEVAR in zone 0, associated with a multibranched thoracoabdominal endograft deployment)...

Intracranial aneurysms in children account for 4%-5% of all cases, with 20% being considered giant (>25 mm). The main sites of occurrence are the internal carotid artery (ICA) and the middle cerebral artery (MCA). Rupture and secondary subarachnoid hemorrhage occur in approximately 55%-72.5% of cases, with a 10%-23% mortality rate. We report the case of a previously healthy nine-year-old girl who developed sudden, severe right retroocular pain and a holocranial headache as a mode of onset. Besides, the patient presented with double vision, and her relatives sought medical attention...

We report the case of an endovascular repair of an aortic arch aneurysm by a surgeon-modified fenestrated endograft with a single fenestration in a high-risk patient unfit for open surgery. A patient of 84 years, chronic ischemic cardiopathic, suffering from prostate adenocarcinoma in chemotherapy treatment, came to our hospital for post-traumatic fracture of the right femur. During the hospitalization, the patient exhibited dysphonia and respiratory disorders for several days, therefore, the patient performed Computed Tomography Angiography (CTA) that found the presence of voluminous aneurysm of the aortic arch with a maximum diameter of about 74 mm...

The aim of this study is to report a case of an unusual large aneurysm of the internal carotid approached with a doubly mandibular osteotomy in order to prove the significance and the excellent result of the specified surgical procedure because of the cooperation of maxillofacial and vascular surgeons. The double mandibular osteotomy is described, as well as the bypass of the aneurysm and the anastomosis of the peripheral edges of the artery. The double mandibular osteotomy can deliver the internal carotid artery from the bifurcation to its entrance to the skull base through the carotid canal and can offer the opportunity to the vascular surgeon to perform the anastomosis easily, quickly and safely...

OBJECTIVE: Agenesis of the internal carotid artery (ICA) is a rare vascular condition that is complicated by intracranial aneurysms and rete mirabile. The altered hemodynamics caused by this distinctive cerebrovascular angioarchitecture can cause ischemic or hemorrhagic accidents. Data on clinical and radiographic features have been limited to describing this vascular pattern. We present five cases of agenesis of the internal carotid artery confirmed by digital subtraction angiography (DSA) and further investigate the influence of altered angioarchitecture on the integrity of intracranial morphology...

A 68-year-old man developed aneurysmal degeneration of the aortic arch and proximal descending aorta after an open ascending graft for a type A aortic dissection. A three-branched endovascular aortic arch repair was performed with patency of all branches despite some degree of initial misalignment of the branches in relation to the target vessels. At 6 months postoperatively, an asymptomatic partial crushing of the left common carotid bridging grafts was observed on computed tomography angiography. This was treated by reinforcing the branch with a balloon-expandable endograft...

Purpose Atherosclerosis is a generalized disorder and can begin to develop in the abdominal aorta by the second decade of life. The nature of these lesions in coronaries and aorta is atheromatous and less sclerotic when compared to peripheral arteries. A broad spectrum of presentations and different types of lesions demand a personalized approach for the best outcome. This study is a case series analysis of major vascular revascularization. We aim to study various revascularization surgeries and underline the wide range of vascular lesions to which it is applied...

Previous randomized clinical trials have so far failed to establish the efficacy of extracranial-intracranial(EC-IC)bypass in the prevention of secondary ischemic strokes. For patients with a recent transient ischemic attack or ischemic stroke ipsilateral to a stenosis or occlusion of the middle cerebral or carotid artery, EC-IC bypass is not recommended as per the American heart association/American stroke association guidelines(Class III, Level of Evidence A). However, patients with severe hemodynamic impairment(misery perfusion)are at high risk of developing stroke, and EC-IC bypass is recommended as per the Japan stroke guideline 2021, provided that the patients show cerebral blood flow less than 80% from baseline and cerebral vascular reserve less than 10%...

BACKGROUND: Endovascular repair of the thoracic aorta (TEVAR) is the preferred option for the treatment of the distal arch and descending thoracic aorta. Fenestrated and branched TEVAR have become an option to treat pathologies of the aortic arch, avoiding sternotomy and cardiopulmonary arrest as well as total surgical debranching. We describe here the case of a symptomatic patient with an arteria lusoria aneurysm associated with Kommerel diverticulum who underwent total endovascular repair with a triple-branched TEVAR...

OBJECTIVES: Preservation of antegrade flow to the left vertebral artery is often achieved by transposition or bypass to the left subclavian artery during zone 2 thoracic endovascular aortic repair (TEVAR). An anomalous left vertebral artery (aLVA) originating directly from the aortic arch is a common arch variant with a reported incidence of 4-6%. In addition, 6-10% of vertebral arteries terminate in a posterior inferior cerebellar artery (PICA), increasing the risk of stroke if not revascularized...

Left subclavian artery esophageal fistula usually occurs after esophageal cancer surgery, which is a rare complication, and it is even rarer after stent implantation of left subclavian artery pseudo-aneurysm. This paper reports the case of a 21-year-old male patient with left subclavian artery pseudo-aneurysm. Two-plus months after stent implantation, he stopped anticoagulant and antiplatelet drugs and developed pain in his left upper limb. The patient was diagnosed with arterial fistula. He was discharged from the hospital successfully after several operations, such as thoracic aortic stent implantation, left common carotid artery left axillary artery artificial vascular bypass...
