COMPARATIVE STUDY
B C Treuren, A Yusuf, D C Galletly, B J Robinson
A comparison was made of the three 22-gauge ported intravenous cannulae available in New Zealand--Venflon 2, Vasofix Braunule and Insyte. All cannulae performed satisfactorily in terms of overall success of cannulation although significant differences were apparent in both clinical and laboratory performance. Anaesthetists' clinical preference was for Insyte and Venflon 2 brands, perceiving these to have greater ease of handling and better packaging. In a series of laboratory studies, Vasofix was found to have the highest flowrate, the fastest time to flashback and the lowest valve opening pressure...

RANDOMIZED CONTROLLED TRIAL

Two hundred and seventy-eight patients scheduled for all types of surgery and premedicated with diazepam and metoclopramide were randomly allocated to one of four groups to compare the relative pain of an injection of 0.25 ml of lignocaine 1% via a 25 gauge needle with the pain of the siting of a 21 gauge Butterfly (Abbott), 23 gauge Butterfly or a 20 gauge Venflon (Vigo Spectramed). The injection of lignocaine and insertion of the 23 gauge Butterfly were associated with the least complaints of pain and least observed responses to pain...

RANDOMIZED CONTROLLED TRIAL
I R Selby, B J Bowles
Three commonly available local anaesthetics were compared, in a controlled trial, for use before venous cannulation. The pain of application of the local anaesthetic, the pain of cannulation, and the rate of successful cannulations were compared. The value of EMLA cream applied for 5 min was questioned. Venous cannulation with a 20G venflon was found to be significantly more painful than the application of any of the local anaesthetics (P < 0.01). Lignocaine 1%, injected subcutaneously, and ethyl chloride spray significantly reduced the pain of venous cannulation (P < 0...

RANDOMIZED CONTROLLED TRIAL
C Richards, L Millar-Jones, M Alfaham
Twelve patients with cystic fibrosis received 12 courses of intravenous antibiotics, each over 10-14 days, both with and without the use of an extended-life disposable filter. The design of the trial was to replace the Venflon cannulae as they became non-patent and inserting a filter on alternate occasions. Thus each patient acted as her/his own control. Comparison of times during which cannulae remained patent showed a 50% improvement with use of a filter for 4 patients and no change for 7 patients. There was no significant difference associated with the use of a filter for the group as a whole but our small sample size excludes modest improvements...

The possibility of a brachial plexus block in children is discussed and presented as a relatively safer method than general anaesthesia. 10 cases aged between 4 and 13 years are reported. Intensive verbal communication with the child during the procedure is essential and sedation may have disadvantages. Prolonged post-operative pain prevention can be achieved by in an indwelling Venflon cannula.

Platelet function was studied during pacing-induced angina pectoris in nine patients with coronary heart disease. Blood was sampled via catheters from the coronary sinus and the aorta at rest and during angina. The influence of the sampling procedures on the platelet function was evaluated in blood collected via catheters and via short venflons. The catheter induced pseudopod formation in the platelets. The aggregation response was similar, while platelet retention as measured with Hellem's method for native blood, was slightly lower in blood collected via catheters than via venflons...

In an experimental in vitro study, it was shown that the rate of infection through the injection route was higher with Venflon cannulae than with Intraflux cannulae (P less than 0.01). A restricted use of cannulae with injection side ports is recommended.

COMPARATIVE STUDY

The rate of contamination of the inner surface of 'Venflon' type cannulae with valved injection sideports was compared with that of 'Intraflux' type cannulae with membranous injection sideports. Of 46 'Venflon' cannulae and 34 'Intraflux' cannulae, 17.4 per cent and 20.6 per cent, respectively, were contaminated and no correlation of contamination of lumen was found with frequency of use of the sideports in the different cannulae, or with duration of use of the cannulae.

Eighty-eight intravenous cannulae with valved injection sideports were examined bacteriologically after approximately three days (64-80 h) normal clinical use. Three methods of culture of the cannulae were used, which distinguished contamination of the outer and inner surfaces as well as detecting bacteria remaining on the inner cannula surface following a washing procedure of the lumen. Of the cannulae sampled, 40.9 per cent were found to be contaminated on their inner surfaces and no correlation was obtained between use of the sideport and contamination of the cannulae at this site...

In an investigation of bacterial colonisation of intravascular cannulae, 16 peripheral venous cannulae of the Venflon variety and 24 chronic haemodialysis cannulae were studied after removal from patients. Studies of colonisation included semi-quantitative microbiological culture and scanning electronmicrographic (SEM) observations. The microbiological findings indicated colonisation of the intravascular portion of the catheter in 4 of 16 Venflon cannulae and 18 of 26 haemodialysis cannulae, largely with skin commensal organisms...

The possibility of employing peripheral venous and peripheral arterial digital subtraction angiography was examined in 351 patients of 65 to 95 years of age with arterial occlusive disease of the lower extremity. After peripheral venous contrast medium injection (using a Venflon needle 1.7-2.0 mm, 40 ml nonionic contrast medium, 20-30 ml NaCl, flow 15/s), conclusive assessment of vascular tone is possible in 86% of the cases. Indication for IV DSA is supplied chiefly by stenoses and occlusions situated proximally or bilaterally in the region of the distal aorta abdominalis, the iliacal, femoral and popliteal arteries...

The possible role of infection through the valved sideport of Venflon cannulae was evaluated by one individual who followed an identical procedure of placing a syringe into the sideport and injecting saline solution. Basic aseptic techniques were used in a contaminated, busy environment. No bacterial growth was found in either anaerobic or aerobic culture media following a total of 1500 injections. The experiment shows that if the measures used could be applied, infection through the sideport would be minimal...

The volume of discard for valid blood sampling from indwelling femoral (leader cath) and radial (Venflon) arterial cannulae (with internal volume of cannula plus connecting tubing of approximately 2 ml) was investigated by analysing sequential 2-ml aliquots of 10-ml samples. The aliquots were analysed for pH, carbon dioxide tension, oxygen tension, standard bicarbonate, haemoglobin concentration, haemoglobin oxygen saturation and potassium concentration. Analysis of variance showed that, for these variables and these catheters, a valid blood sample was obtained after discarding 4 ml, but not after only 2 ml...
