About ten years ago in Taiwan, the government initiated early intervention for development-delayed children. For the purpose of early detection of latent cases and providing early intervention at the prime time for children, nurses are involved in the service, and their main contribution is in the screening procedure. Early intervention emphasizes the family-centered approach, the philosophy of which is the same as that of family nursing. With similar concerns as its axis, this article discusses the meaning and essential concepts or elements of early intervention practice, including the family assessment and individual family service plan (IFSP), home-based early intervention, family support, parent participation, parent education, etc...

Children and adolescents with chronic diseases and disabling conditions often need educationally related services. As medical home providers, physicians and other health care professionals can assist children, adolescents, and their families with the complex federal, state, and local laws, regulations, and systems associated with these services. Expanded roles for physicians and other health care professionals in individualized family service plan, individualized education plan, and Section 504 plan development and implementation are recommended...

AIM: The aim of this study was to determine the role of combined mechanical and pharmacological prophylaxis in the prevention of deep venous thrombosis (DVT) after total knee replacement (TKR). DESIGN: prospective case series study. METHODS: Between October 2002 and June 2003, 38 total knee procedures were carried out on 34 patients (4 patients had bilateral TKR). To exclude the presence of a concomitant DVT echo-color-flow of the legs was performed between 2 and 1 week prior to surgery, in the postoperative period (before discharging) and 30 days after surgery...

This paper describes a study on the sustainability of Community-based Animal Health Worker (CAHW) services in Mwingi District, Kenya. These services began in 1992 and were supported by the District Veterinary Authority (DVA) with assistance from the Integrated Food Security Programme-Eastern (IFSP-E). Over time and using a process of participatory reviews with multiple stakeholders, the system evolved into a network of CAHWs. The study focused on CAHWs' service sustainability and their relationships with other animal health service providers...

The Individual Education Plan and Individual Family Service Plan are legally mandated documents developed by a multidisciplinary team assessment that specifies goals and services for each child eligible for special educational services or early intervention services. Pediatricians need to be knowledgeable of federal, state, and local requirements; establish linkages with early intervention, educational professionals, and parent support groups; and collaborate with the team working with individual children.

In summary, newborn intensive care is a rapidly changing field serving a heterogeneous population of infants and families. A family-centered/developmental perspective needs to be incorporated as a part of basic nursery care. Some infants and families have developmental needs beyond this baseline. The ability to respond to these needs is supported by PL 99-457. A way to sort out which infants and families might benefit from entering the IFSP process has been described. The IFSP process and plan have been presented and a framework for integrating this continuum of care into the NICU has been proposed...

Health care social workers are increasingly providing services to young children who are technology-dependent through participation on individualized family service plan (IFSP) teams, as well as in clinical settings. Such participation suggests that new models for assessing the impact of medical technologies on family functioning will, of necessity, also begin to emerge. Assessment implications from a family systems perspective are presented that may be used to guide the development of future assessment strategies for health care social workers...

Since positive family-child relationships are initially established during the first 3 years, it is imperative that service providers focus their efforts on the family unit, as well as on the child. The Individuals with Disabilities Education Act--Part H, as amended (IDEA--Part H), supports this concept and has mandated development of an IFSP for each infant and toddler and his or her family eligible for early intervention services. The confirmed diagnosis of hearing loss for a child may have long-term effects on the family...

We used a noninvasive monitor of arterial pressure to determine whether respiratory changes in arterial pressure were closely correlated with airflow obstruction in asthmatic patients during bronchial challenge with methacholine. To validate the noninvasive measurement of respiratory changes in arterial pressure, a preliminary study in 6 subjects with normal cardiovascular and respiratory systems was done during cardiac catheterization for suspected coronary artery disease. There were no significant differences between inspiratory falls in systolic pressure measured noninvasively and those measured from intraaortic pressure...

Hawaii's system of prevention and early intervention embodies Part H principles that a child with a disability is first surrounded by a family, then by a community. Since 1986, families in Hawaii have been active in early intervention, first by persuading the governor to name the Department of Health as the lead agency, and then by writing much of the original grant application, interviewing and selecting staff members, and serving on the Hawaii Early Intervention Coordinating Council and its subcommittees...

PL 99-457 mandates nutrition as one of eight disciplines to be included on the team providing services to handicapped infants (up to age 3) and their families. A case-managed range of services is required to enable the child to benefit from early interventions. Findings from a survey of a sample of entry-level Plan IV/V Programs, Coordinated Programs in Dietetics, and dietetic internships randomly selected from The American Dietetic Association's 1988 Directory of Dietetic Programs indicate that the growth and development of handicapped infants and their nutrition care are underexposed in most didactic and experiential components of the education of entry-level dietitians...

By requiring a family focus in the provision of early intervention services, the Individualized Family Service Plan (IFSP) provisions of Public Law 99-457 establish a new precedent in public policies for families of children with disabilities. In this article, the conceptual and procedural mandates of the IFSP are analyzed from two perspectives: as bold, radical steps taken by the federal government and as logical extensions of current service delivery practices that focus on the child but involve parents in planning and implementing programs...

A hospital-based multidisciplinary team is demonstrating services for medically fragile infants which facilitate transition from the hospital to the community and home. The team provides education and counseling for communities and families to overcome barriers to services. With individualized case management, the team facilitates family access to the community services they need in infant health, infant development and family functioning. The team plans the infant's discharge from the hospital and collaborates on the initial Individualized Family Service Plan (IFSP) with the family and local community providers...

The individualized education plan (IEP) and the individualized family service plan (IFSP) are mandated for children with special needs. Occupational therapists participate in the development of both the IEP and the IFSP. This paper summarizes the similarities and the differences in the mandated components. The components addressed are (a) information about the child's status, (b) information about the family, (c) outcomes for the child and family, (d) intervention services, (e) other services, (f) dates and duration of services, (g) selection of a case manager, and (h) transition plans...

Services for developmentally delayed children from birth to age three consider the family first. Eligibility for services is determined through a multidisciplinary assessment. Once a child qualifies for service, a multidisciplinary team that includes the family develops an IFSP. The SLP may serve as the service coordinator for the plan or as a team member. The plans must contain specific information that includes documentation of current status and major outcomes for the coming year. An SLP may find that contributing effectively to an IFSP requires new competencies...
