BACKGROUND: Dense Deposit Disease is a rare condition affecting the Bruch's membrane and the glomerular basement membrane. We report the progression of the ocular manifestations over a 30 year follow up period, longer than any previous report. CASE PRESENTATION: A 44 year old male presented with pigmentary changes at the macula noted by his optician. Best corrected visual acuity at presentation was good in both eyes. Fundoscopy showed pigmentary changes and drusen, and investigation using intravenous fundus fluorescein angiography did not demonstrate any choroidal neovascular membrane...

A 78-year-old woman was referred by an optician for an incidental finding of peripapillary naevus in the absence of visual symptoms. On examination, the right eye showed a choroidal naevus of around 1.5 disc diameter, adjacent to the optic disc. Optical coherence tomography detected the presence of pigment epithelium detachment and subretinal fluid within the context of the naevus. Fundus fluorescein angiography and indocyanine green showed multiple polypoidal lesions (Polypoidal Choroidal Vasculopathy, PCV) in the choroid within the spectrum of a pachychoroid, without any sign of malignancy related to neovascularisation...

Purpose The purpose of this paper is to assess if the GOC considers relevant factors at all stages of its deliberations into misconduct, as required by the determinations in the cases of Cohen, Zygmunt, and Azzam, and to assess whether those circumstances described in the Hearings Guidance and Indicative Sanctions as warranting removal of an optician from the relevant registers lead to that outcome. Design/methodology/approach The consideration of specific factors in determining impairment of fitness to practise was compared with their subsequent consideration when determining the severity of sanction...

Edge effect is regarded as one of the most difficult technical issues in a computer controlled optical surfacing (CCOS) process. Traditional opticians have to even up the consequences of the two following cases. Operating CCOS in a large overhang condition affects the accuracy of material removal, while in a small overhang condition, it achieves a more accurate performance, but leaves a narrow rolled-up edge, which takes time and effort to remove. In order to control the edge residuals in the latter case, we present a new concept of the 'heterocercal' tool influence function (TIF)...

In brief: Eyesight and clarity of vision are not the only aspects of eye care for athletes. Today's eye care professionals-the opticians, ophthalmologists, and optometrists-must be able to help athletes enhance their performance not only with proper evaluation of visual acuity, but also by providing the most appropriate means of correction and protection for the sport involved. This panel of eye care experts discussed standardized testing, mandatory examinations, screening, types of eye protection for specific sports, and education needs for eye care professionals, coaches, trainers, and athletes...

The 2001 NHG practice guideline on 'Refraction disorders' was revised for the second time in October 2015. The title has been changed to 'Visual symptoms', since besides refraction disorders other conditions such as glaucoma, macular degeneration, cataract and acute eye symptoms are also covered. The ophthalmologist is the specialist to whom GPs refer patients most often. Apart from the GP, ophthalmologist and youth health care physician, various other allied health professions are involved in eye healthcare...

PURPOSE: This report presents a case, which highlights key principles in the pathophysiology of macular holes. It has been hypothesized that anteroposterior (AP) and tangential vitreous traction on the fovea are the primary underlying factors causing macular holes [Nischal and Pearson; in Kanski and Bowling: Clinical Ophthalmology: A Systemic Approach, 2011, pp 629-631]. Spectral domain optical coherence tomography (OCT) has subsequently corroborated this theory in part but shown that AP vitreofoveal traction is the more common scenario [Steel and Lotery: Eye 2013;27:1-21]...

BACKGROUND: People with dementia are susceptible to adverse drug reactions (ADRs). However, they are not always closely monitored for potential problems relating to their medicines: structured nurse-led ADR Profiles have the potential to address this care gap. We aimed to assess the number and nature of clinical problems identified and addressed and changes in prescribing following introduction of nurse-led medicines' monitoring. DESIGN: Pragmatic cohort stepped-wedge cluster Randomised Controlled Trial (RCT) of structured nurse-led medicines' monitoring versus usual care...

PURPOSE: Head movements in older people may contribute to their dizziness and equilibrium problems. Head gain is the ratio of head movement to total movement (head + eye) when executing a saccade to an eccentric target. Two studies have investigated the relationship between head gain and age but have provided conflicting results. METHODS: We report head gain data collected from research laboratories and optician stores. Our sample sizes are much larger (n = 657 for laboratory, n = 64,458 for optician stores), permitting more detailed analyses...
