The lunate foveola is often the starting point of a cystic swelling called a "ganglion". To make the anatomy of this region more relevant to the needs of clinicians and more accessible to students, we propose to introduce the term lunate foveola (Foveola lunata) on the dorsal side of the wrist just distal to the os lunatum. The easily located foveola will help in the examination of the wrist to more easily understand the anatomy of the wrist and facilitate the examination of patients with wrist injuries when an injury to the lunate (Os lunatum) is suspected...

Lunate dislocations are complex and uncommon wrist injuries, often resulting from high-energy trauma like road traffic accidents, falls, and industrial mishaps. Timely diagnosis and appropriate surgical management are critical to achieving favorable outcomes and minimizing long-term disability. Here, we present a case of a 22-year-old male who sustained a trans-scaphoid lunate fracture dislocation due to a motorcycle accident. Prompt wrist x-ray evaluation and subsequent surgical interventions led to the successful restoration of wrist function and stability...

Carpal dislocations result from sequential disruption of the complex relationship between the bones and ligaments of the wrist. Injuries to the carpus occur via predictable mechanisms, an understanding of which is critical to identify and treat these frequently missed patterns of injury and to avoid the sequela of chronic instability. Lunate dislocations are by far the most common, but isolated dislocation of other carpal bones can also occur. Open reduction and internal fixation still remains the gold standard for treatment regardless of the debate around the specific approaches...

Herein we present a case of four simultaneous dislocations of the hand resulting from a motorcycle accident in a young man. In this case, the 1st CMC, MCP, and IP joints of the right hand were dislocated along with right perilunate dislocation. Perilunate dislocation was treated with open reduction and internal fixation. Close reduction of IP and CMC was done and CMC was fixed by pins. MCP dislocation was treated by open reduction and pinning in addition to collateral ligament and capsular repair. In follow-up excellent functional activity and range of motion were observed...

INTRODUCTION: Perilunate dislocations are uncommon injuries requiring high-energy trauma. They are responsible for serious lesions that can compromise the function of the limb. We report a case of pure bilateral perilunate dislocation in a parachutist while landing. CASE REPORT: The case involved a 30-year-old military officer of special forces who underwent closed trauma to both wrists following an automatic opening parachute jump. He had first consulted a garrison medical center where a wrist sprain had been treated...

CASE: A 26-year-old right-hand-dominant man sustained a left transradial, translunate perilunate injury after motor vehicle collision. The proximal lunate fractured a primarily cartilaginous 15 × 15 mm osteochondral shear fragment with 40% articular surface involvement. A novel lunate fracture suture anchor fixation technique is described, which led to fracture healing. The radial styloid was treated with a single screw. CONCLUSION: Translunate perilunate injuries involving the lunate proximal articular surface are rare...

Background: In patients with perilunate injuries (PLI) with multiple ligamentous and bony injuries involving the proximal carpal row, open reduction and internal fixation (ORIF) can be difficult and lead to poor functional outcomes. Proximal row carpectomy (PRC) is an alternative procedure that has been used for severely comminuted fractures. The aim of our study is to evaluate the long-term functional outcome (minimum 5 years) of patients that underwent an emergency PRC for PLI. Methods: We conducted a retrospective study of patients who underwent PRC at our centre between 2001 and 2016...

Delayed or missed diagnosis of perilunate or lunate dislocations can lead to significant morbidity. Advances in computer vision provide an opportunity to improve diagnostic performance. In this study, a deep learning algorithm was utilized for detection of perilunate and lunate dislocations on lateral wrist radiographs. A total of 435 lateral wrist radiographs were labeled as normal or pathologic (perilunate or lunate dislocation). The lunate in each radiograph was segmented with a rectangular bounding box...

INTRODUCTION: Perilunate dislocations and perilunate fracture-dislocations (PLFD) are relatively uncommon injuries, comprising <10% of wrist injuries. Perilunate injuries are often complicated by median neuropathy reported in 23-45% of cases, whereas there are very few reported cases of associated ulnar neuropathy. Combined greater arc and inferior arc injuries are also rare. We report an unusual PLFD pattern with associated inferior arc injury and acute ulnar nerve compression. CASE REPORT: A 34-year-old male sustained a wrist injury after a motorcycle collision...

PURPOSE: Distal radius fractures are associated with a high incidence of concomitant soft tissue injuries, including lesions of the triangular fibrocartilage complex and intercarpal ligaments. While advanced imaging has allowed for greater identification of such tears, discerning which lesions pose a functional consequence remains a challenge for the hand surgeon. A review and guideline for arthroscopic evaluation of suspected combined injuries is presented. RESULTS: Arthroscopic evaluation of distal radius fractures provides several unique advantages in such instances...

Perilunate dislocations and fracture-dislocations are considered rare injuries. Perilunate injuries are frequently missed during primary evaluations. We report a case of a 37-year-old male presenting with an open perilunate fracture-dislocation a few days after experiencing trauma. He underwent repeated debridements, and provisional external fixator application followed by a definitive open reduction through a combined dual approach and internal fixation of scaphoid and capitate with headless screws. Aggressive physiotherapy exercises were started eight weeks after definitive fixation...

BACKGROUND: Treatments for chronic perilunate or lunate dislocations are very difficult and associated with poor prognoses. There is no established treatment method and are still many controversies. CASE PRESENTATION: We reported three cases of chronic neglected lunate volar dislocation treated with a novel surgical technique. All three cases were males with wrist pain and tingling sensation. Radiographs confirmed chronic volar dislocation of the lunate. Open reduction was performed by combined volar and dorsal approaches...

Lunate and perilunate dislocations, although somewhat uncommon, are serious injuries and can be easily missed. The mechanism of injury is hyperextension of the wrist, often associated with a fall on the outstretched hand (FOOSH) injury, falls from height, or motor vehicle crash with a high-energy traumatic injury to the wrist. Perilunate dislocations typically present with pain and swelling over both the dorsal and volar aspects of the wrist and limited range of motion of the wrist. Perilunate dislocations result in disruption of the relationship between the lunate and the capitate, whereas lunate dislocations result in a disruption of the lunate bone, both from the radius and the capitate, primarily diagnosed on the lateral view of the wrist radiograph...

We systematically reviewed the incidence of complications and outcomes of different surgical methods for acute perilunate injury in the MEDLINE, Scopus, Embase and Cochrane Library databases. Forty-three articles with 880 patients were included. The most common complications were arthritis (30%), carpal instability (15%), avascular necrosis of the lunate (12%), complex regional pain syndrome (11%), and nonunion or avascular necrosis of the scaphoid (9%). In the meta-analysis, the mean scapholunate gap was 1...

Because of their low incidence, studies about carpal fractures are rare. The aim of the present study was to analyze epidemiology and treatment of fractured carpal bones. We retrospectively analyzed data of 178 patients admitted to our emergency room with carpal fractures over 6 years. More males than woman were injured. In 91%, a CT scan was performed. The most commonly affected bone was the triquetrum followed by the scaphoid. Almost all triquetral fractures were treated conservatively as opposed to perilunate dislocations that were all operated on...
