Although relatively rare, surgical instrument retention inside a patient following central venous catheterization still presents a significant risk. The research presented here compared two approaches to help reduce retention risk: Bow-Tie Analysis and Systems-Theoretic Accident Model and Processes. Each method was undertaken separately and then the results of the two approaches were compared and combined. Ca2Mn3O8 exhibits a complex layered structure comprised of Mn3O8(4-) layers separated by Ca(2+) ions. In contrast with the more traditional triangular delafossite layered materials the Mn3O8(4-) layers additionally exhibit an ordered vacancy, which forms a 'bow-tie' like arrangement of the Mn(4+) ions. We report a comprehensive study of the magnetic properties of a series of Ca2Mn3O8 materials with different morphologies. High-sensitivity distributed measurement of hydrostatic pressure is experimentally demonstrated by optical time-domain analysis of Brillouin dynamic grating (BDG) in polarization maintaining fibers (PMF's). The spectral shift of the BDG in four different types of PMF's are investigated under hydrostatic pressure variation from 14.5 psi (1 bar) to 884.7 psi (61 bar) with less than 2 m spatial resolution.
