A 27-year-old man referred to our department with a 1-month history of intermittent melena and dizziness. Gastroscopy and colonoscopy did not identify the site of bleeding. Laboratory data showed hemoglobin 6.6g/dL. Video capsule endoscopy (CE, Anhon,China) and antegrade double-ballon enteroscopy (DBE, EN450P5, Fujinon, Japan) were carried out and a space occupying lesion in the proximal jejunum was identified (Fig.1). Since its introduction in 2001 capsule endoscopy opened up the small bowel for diagnostic approaches followed by double balloon enteroscopy which enabled the endoscopic community to perform therapeutic interventions in the whole small intestine. In this review the scientific developments related to indications, diagnostic yield and complications of the last years between the competing devices double ballon enteroscopy, single balloon enteroscopy and spiral enteroscopy are illustrated.

It was done 192 double-ballon enteroscopies in 141 patients at the period from March 2009 to March 2012. Epithelial benign tumors were detected in 10 (7.1%) patients with congenital polypoid syndromes. Endoscopic excision of polyps was done in 6 patients including 1 case with laparoscopic-assisted technique. Urgent open surgeries were performed before this in 4 patients with Peutz-Jeghers' syndrome and invaginated small bowel obstruction. Sclerosing mesenteritis is a rare, benign, and chronic fibrosing inflammatory disease of the mesenteric fatty tissue. Its aetiology is unknown. In the present report we describe a 56-year-old women who presented with postprandial abdominal pain, and weight loss. The present case illustrates a patient with obscure gastrointestinal bleeding which MiroCam capsule endoscopy showed erosions and ulcers from distal jejunum to ileum. Considering that ileal ulcers where closer to ileocecal valve, ileoscopy was repeated. Diagnosis with new biopsies was Crohn disease.
