REVIEW
Oliver Moeschler, Michael Karl Mueller
Since its introduction in 2001 capsule endoscopy opened up the small bowel for diagnostic approaches followed by double balloon enteroscopy which enabled the endoscopic community to perform therapeutic interventions in the whole small intestine. In this review the scientific developments related to indications, diagnostic yield and complications of the last years between the competing devices double ballon enteroscopy, single balloon enteroscopy and spiral enteroscopy are illustrated.

It was done 192 double-ballon enteroscopies in 141 patients at the period from March 2009 to March 2012. Epithelial benign tumors were detected in 10 (7.1%) patients with congenital polypoid syndromes. Endoscopic excision of polyps was done in 6 patients including 1 case with laparoscopic-assisted technique. Urgent open surgeries were performed before this in 4 patients with Peutz-Jeghers' syndrome and invaginated small bowel obstruction. The authors consider that double-ballon enteroscopy is highly informative diagnostic and treatment method in case of small bowel diseases...

BACKGROUND/AIMS: Double balloon enteroscopy (DBE)-assisted endoscopic retrograde cholangiopancreatography (ERCP) is frequently performed in patients with surgically altered intestinal anatomy. Moreover, it is also utilized in some cases with pancreatic indications, particularly patients with Roux-en-Y anatomy following Whipple’s procedure or pylorus-preserving pancreaticoduodenectomy. Here, we present our experiences using DBE-assisted ERCP in patients with Roux-en-Y anatomy, with a focus on pancreatic indications...

Sclerosing mesenteritis is a rare, benign, and chronic fibrosing inflammatory disease of the mesenteric fatty tissue. Its aetiology is unknown. In the present report we describe a 56-year-old women who presented with postprandial abdominal pain, and weight loss. Ultrasound, computed tomography, and magnetic resonance imaging revealed a mesenteric mass of 15 cm. The findings were typical for this disease. Additionally the patient underwent a single ballon enteroscopy in which the mucosa showed a considerable hyperergic reaction...

INTRODUCTION: Peutz-Jeghers syndrome (PJS) is a familial polyposis syndrome characterized by multiple hamartomatous polyps throughout the gastrointestinal tract. The aim of our study was to retrospectively determine the diagnostic value of MR enterography (MRE), performed in supine and prone position, in the detection of small bowel polyps in PJ patients. MATERIALS AND METHODS: We retrospectively reviewed MRE examinations of 8 PJS patients who underwent MRE, pushed-double-ballon enteroscopy, laparoscopic endoscopy or surgery, within 3 months...

The present case illustrates a patient with obscure gastrointestinal bleeding which MiroCam capsule endoscopy showed erosions and ulcers from distal jejunum to ileum. Considering that ileal ulcers where closer to ileocecal valve, ileoscopy was repeated. Diagnosis with new biopsies was Crohn disease. Capsule endoscopy is an innovative technology, which let us have high resolution color pictures through gastrointestinal tract for more effective and diagnostic view of small bowel in a minimal invasive way. New guidelines in obscure GI bleeding agreed that initial diagnostic imaging of the small bowel should be performed using capsule endoscopy...

Diagnosis of and therapy for small bowl disease is a new challenge for modern endoscopy. Following the development of capsule endoscopy it is possible to inspect the whole small intestine. Therefore one needs adequate endoscopes in order to take tissue samples and perform the corresponding therapeutic measures. Today, the technical procedure of double-ballon enteroscopy is established and is available for routine clinical use. Double-ballon enteroscopy is usable both perorally and peranally, and can be used to inspect the entire small intestine...
