The authors present a 56-year-old-man with idiopathic glossopharyngeal neuralgia, complicated by cardiac disturbance with asystole and complex partial seizures. The mechanism of epileptic seizures was released due to transient brain ischaemia, which was caused by cardiac disturbance with asystole, during pain episodes. Pacemarker implantation completely abolished cardiac disturbance and complex partial seizures.

For estimation of light radiomodification and cellular neuroadaptation of the circadian pacemarker, the morphological changes of suprachiasmatic nuclei (SCN) neurons of rat hypothalamus after 48-hours bright light exposure, the single 5 Gy whole-body X-irradiation and their combination were subjected to analysis. We report a rare incident of complete heart block occurring after induction of general anesthesia in an apparently fit patient. An external temporary pacemarker was first applied, which was then replaced by isoproterenol infusion, but permanent pacing was required postoperatively. The role of anesthesia as a precipitating factor was discussed. Carotid sinus hypersensitivity (CSH) is a common cause of syncope, and permanent pacemarker is unequivocally indicated in such patients. 489 operation were performed at 323 patients with transvenous implantation of cardiac pacemarkers in 1968-1973. The average of the complications is coming to 25 p.c. of all operations. 41 (34 p.c.) operations of correction are caused by a dislocation of the electrode, 39 (33 p.c.) by a break of the electrode, 18 (14 p.c.) by infectious complications. A description is given of the introduction of an epicardial screw-in electrode (Medtronic, model 6917) by the infrasternal approach in 22 patients with severe cardiac conduction disorders. Special attention is given to the surgical technique, the electrode and the measurement of the threshold value. Comparison of the various techniques of pacemarker implantation leads to the conclusion that this method of treatment offers many advantages.

To study the role of the suprachiasmatic nuclei (SCN) in generating circadian rhythms in female rats, lesions were placed in the SCN or in the medial preoptic (PO) region. Serial blood sampling at 4-hour intervals at 3 and 13 weeks after surgery indicated that complete SCN destruction abolished rhythmic fluctuation in plasma corticosterone levels in individual rats. Partial destruction produced less interference, while medial PO lesions that spared the SCN were without effect. Similar effects were noted on daily changes in body temperature at 10 weeks after surgery; however, some rats showed evidence of dissociation of these two rhythmic functions in that some lesions appeared to affect one and not the other...

A Helifix electrode was inserted transvenously in the right atrial appendage for temporary atrial stimulation in three patients. The low stimulation threshold as well as the excellent stability of the electrode encouraged us to proceed to permanent implantation of this lead in thirteen patients. The lead was introduced through the saphenous vein in twelve patients and through the cephalic vein in one patient.
