COMPARATIVE STUDY

Serous zinc levels and electrogustometry were studied on thirty patients undergoing periodic hemodialysis. After dialysis a better electrogustometric threshold (p less than 0,05) and also higher serous zinc levels were found. The two parameters showed no significant correlation. These results are suggestive of a toxical damage of taste receptors in the chronic renal failure.

A male patient undergoing hemodialysis in whom the cause of severe hyperkalemia (more than 8 mmol/liter) was found to be an unusual pica is described. His pica consisted of ingestion of burnt match heads (cautopyreiophagia). The potassium content of his daily consumption of burnt matches added 80 mmol to his 45 mmol dietary potassium intake. Hypogeusia and hypozincemia were documented. Oral zinc supplementation (zinc sulfate 220 mg per day for 10 weeks) normalized his plasma zinc level and taste acuity. Dialysis patients generally have a poor appetite, do not enjoy eating, and complain of food, particularly protein, as being disagreeable. Twenty dialysis patients with the above symptoms were tested for taste acuity, serum zinc (Zn), Zn concentrations in hair samples (intracellular Zn), and daily caloric intake. A double-blind, cross-over study was instituted using a Zn supplement and a placebo. After supplementation with Zn, taste acuity markedly improved in 95% of patients and Zn concentrations in hair increased in 85% of patients...
