Intraabdominal infection by Actinomyces species, although a rare condition, usually occurs after a disruption of the mucosal barrier in a peritoneal organ. This infection is characterized by the development of an extended and persistent inflammatory and fibrotic reaction that can be mistaken for other pathogens or different etiologies, like tumors or inflammatory diseases. It can present as an abscess, a stricturing tissue with multiple adhesions, and/or a fistulization. Early diagnosis, targeted and prolonged antimicrobial therapy, and optimal drainage when indicated, are the key to success...

BACKGROUND: Although most postmenopausal women diagnosed with endometrial cancer usually present with vaginal bleeding, when complete cervical stenosis is present, this sign may be missing. In these cases, the patient usually complaints for pelvic or abdominal pain while the transvaginal ultrasonography might reveal the presence of an intrauterine fluid collection in association with a thickened endometrial lining. CASE REPORT: We present the case of a 65-year-old patient who presented with association of pelvic pain, enlarged uterine cavity with an underlying hematometra and an irregular, thickened endometrium who was submitted to surgery for total histerectomy, bilateral adnexectomy, pelvic and para-aortic lymph node dissection...

OBJECTIVE: The aim of this study was to determine the incidence and evolution of the atypical glandular cells of undetermined significance (AGUS) without associated lesions identified on cervical Pap smear. MATERIAL AND METHODS: Retrospective study concerning 33.923 cervicovaginal smears performed in the Cytopathology Department of the Coimbra University Hospital (H.U.C.) between January 1995 an December 1997. AGUS diagnosis, isolated or associated with other lesions, was made in 716 cases...

OBJECTIVES: To compare surgical morbidity and time, as well as anatomical outcomes between vaginal histerectomy and uterine preservation in the treatment of uterine prolapse using a mesh kit (Nazca). METHODS: Randomized controled trial with 31 women with uterine prolapse POP-Q stage 3 or 4 pelvic organ prolapse who underwent vaginal surgery using tipe I polypropilene mesh (Nazca). They were randomized in two groups: group HV: hysterectomy and pelvic reconstruction floor with mesh (n=15); group HP: hysteropexy and pelvic reconstruction floor with mesh (n=16)...

OBJECTIVE: To evaluate the incidence, risk factors, indications, outcomes and complications of emergency peripartum hysterectomy performed after cesarean and vaginal deliveries. METHOD(S): We analyzed retrospectively 28 cases of emergency peripartum hysterectomy operations performed between February 2001 and February 2007 at the Istanbul Goztepe Training and Research Hospital, which is a teaching hospital operating under the Turkish Ministry of Health. The indications, risk factors and the associated complications were compared with control groups...

OBJECTIVES: To evaluate the efficacy and safety of Tension-free vaginal tape (TVT) for treatment of female stress urinary incontinence (SUI). To determine the mid-term outcome of TVT performed during other pelvic floor reconstructive procedures. PATIENTS AND METHODS: 100 women with SUI undergoing TVT procedure under spinal anesthesia from January 2000 to November 2002 were studied. 76 women were treated with TVT alone. 24 patients were treated with TVT and pelvic floor reconstruction: we repaired 24 cystoceles grade II-III, 3 rectoceles and 4 concomitant vaginal histerectomies...

UNLABELLED: The abdominal histerectomy has been used during several years as a definitive treatment for the high grade squamous intraepithelial lesion of the cervix, because it is associated with a high index cure, with 1% to 15% of relapses as neoplastic lesions in the vaginal stump or isolated lesions. The object of this study is to evaluate the extrafascial abdominal histerectomy as an effective and definitive therapeutitic method for the high grade squamous intraepithelial lesion, with an annual colpocythologic follow up...

This paper reports an analysis of 217 patients with invasive carcinoma of the cervix, treated with radical histerectomy at The Oncology Service, General Hospital of Mexico. The commonest complications of surgical procedure, were related with the urinary tract: atony bladder observed in 27.6% of patients, vesico-vaginal fistulas in 7.8% and uretero-vaginal fistulas, in 5.5%. Lymph node metastases were present in 20/143, (13.9%) patients classified as belonging to stage Ib and 10/51, (19.6%) of stage IIa, p less than 0...

From 1968 to 1975 the authors performed 632 vaginal histerectomies; morcellement was applied in 81 patients (12.81%). The largest uterus subjected to morcellement weighed 700 g; most of them weighed about 250 g. In all cases morcellement was applied due to the enlarged uterus. In the authors opinion, the uterus can be extirpated vaginally until it reaches the size corresponding to pregnancy up to 12 weeks. The postoperative morbidity rate in patients with vaginal histerectomy and morcellement proved to be 30...

The authors analyse 125 cases of vaginal histerectomy (V.H.) preceded by vaginal and abdominal operations (15.3% of a total of 817 V.H.) There were 95 cases of previous abdominal operations and 30 cases of previous gynecological operations (19 vaginal and 11 abdominal). The most frequent previously performed vaginal operation was anterior and posterior colporrhaphy. The authors have come to the conclusion that neither previous abdominal nor previous gynecological surgical interventions make the carrying out of vaginal histerectomy more difficult...
