Contact allergy to skin disinfectants is not often recognized in children. We report the cases of six children (1-16.5 years old) with allergic contact dermatitis to ingredients commonly contained in commercial antiseptic and cosmetic products. Patch test responses to chlorhexidine, benzyl alcohol, and benzalkonium chloride varied from one child to another one, but most children were sensitized to at least two components. In several of the cases, exposure had initially occurred in the neonatal period, but diagnosis occurred only after multiple reactions of increasing severity...

PURPOSE: To investigate whether preoperative skin cleaning of the chin, neck, and chest with chlorhexidine soap could reduce suture contamination by Cutibacterium acnes (C. acnes) in patients undergoing arthroscopic rotator cuff repair. METHODS: This study included patients who underwent primary arthroscopic rotator cuff repair. Exclusion criteria included age <18 years, previous shoulder surgery, history of shoulder infection, and allergy to chlorhexidine. Patients were randomized into three groups...

BACKGROUND: Although chlorhexidine allergy has been shown to be mediated by immunoglobulin (Ig) E, few reports investigated the mechanism of chlorhexidine-induced anaphylaxis using basophil activation tests (BATs). CASE PRESENTATION: A 79-year-old man underwent cholecystectomy under general anesthesia. Anaphylaxis was diagnosed based on the clinical symptoms and high serum tryptase and histamine levels. Skin tests showed positive results only for chlorhexidine. Subsequently, BATs demonstrated that the causative agent was likely chlorhexidine...

Perioperative anaphylaxis occurs in about 1:6000 of all surgical procedures. Recently, the pathophysiology of anaphylactic reactions via mast cell related G-protein und complement-activated pseudo-allergy have been added to the "old" IgE mediated etiology. New culprits for anaphylactic reactions are chlorhexidine or gelatine as hemostypticum or blue surgical dyes to mark the situs. Biphasic anaphylactic reactions should be kept in mind. In the meantime, propofol use is allowed in egg and soybean allergic patients...

OBJECTIVE: Chlorhexidine is the gold standard for chemical biofilm control in dentistry. As with all therapeutic agents, side effects should be considered, which may include allergic reactions. The present review focused on the allergy-causing potential of dental CHX mouthrinses. METHOD AND MATERIALS: Four databases were searched for systematic reviews related to chlorhexidine and dentistry (MEDLINE, Embase, Web of Science, and Scopus). Original papers available in the identified studies and meta-analyses were individually screened for allergy by both authors...

Background - Topical glucocorticoids commonly are used in the management of canine atopic dermatitis to control and prevent allergy flares. Compounding commercial veterinary wipe/pad products to include dexamethasone sodium phosphate (Dex SP) can simplify treatment protocols for owners. Dex SP has not been evaluated for stability when added to wipes/pads. Hypothesis/objective - To evaluate the stability of Dex SP when compounded in three commercial veterinary wipe/pad products containing chlorhexidine. Methods and materials - Dex SP (Dexium, Bimeda; Oakbrook Terrace, IL, USA) was added to TrizCHLOR 4 wipes (TCL; Dechra Veterinary Products; Overland Park, KS, USA), KetoHex wipes (KH; VetOne; Boise, ID, USA), and DOUXO Chlorhexidine Pads (DCP; Ceva Animal Health; Lenexa, KS, USA), creating a 0...

OBJECTIVE: To determine modifiable risk factors associated with surgical site infection (SSI) and postpartum endometritis. We hypothesized that inappropriate surgical antibiotic prophylaxis would be a risk factor for both types of infections. METHODS: This was a single-center case-control study of SSI and endometritis after cesarean delivery over a two-year period from 2016 to 2017. Cases were identified by ICD-10 diagnosis codes, infection control surveillance, and electronic medical records search, and were subsequently confirmed by chart review...

Introduction: High infection rates after cesarean section are a major concern in the United States. A majority of antiseptics have irritant properties if they are not used as specified in companies' guidelines. Allergic contact dermatitis caused by povidone‑iodine is infrequent and is misdiagnosed by clinicians who fail to differentiate allergy from irritation. This case report describes an allergic reaction to povidone‑iodine after cesarean section. Case Presentation: A 28-year-old underwent a repeat low transverse cesarean-section at 39 weeks of gestation...

OBJECTIVE: The oral cavity undergoes frequent stress caused by repeated mechanical trauma, and the constant contact of the injured oral mucosa with bacteria leads to the production of various pro-inflammatory cytokines and chemokines. Neutrophils play essential roles in the acute inflammation against the invasive microbiota, and compromised neutrophil recruitment hinders the bacterial clearance and worsens periodontitis. In this study, we aimed to explore whether wounding at the oral cavity would have an impact on the neutrophil lineage, and, if so, whether microbial contamination of the wounded surface plays significant roles...

It is of utmost importance that the culprit agent is identified and avoided to allow continuation of dialysis treatment as needed. BACKGROUND: Although povidone-iodine (iodine) is the only Food and Drug Administration-approved vaginal antiseptic solution, there is a lack of comparative data evaluating alternatives. Chlorhexidine gluconate is readily accessible, recommended by multiple societies as an alternative for patients with iodine allergy, and preliminary data indicate that it may provide superior antisepsis. BACKGROUND: Perioperative anaphylaxis is a severe immediate hypersensitivity reaction to drugs administered in immediate temporal association to surgical procedures. The European Academy of Allergy and Clinical Immunology recommends allergologic tests be performed within the golden period of between 1 and 4 months after the date of the event to avoid false negatives. Nonetheless, many obstacles prevent patients from receiving diagnostic tests within the recommended time frame. The diagnosis of a drug hypersensitivity reaction (DHR) is complex. The first step after taking the clinical history is to look for a sensitization to confirm or exclude the diagnosis and to identify the culprit drug. Skin tests are the primary means of detecting sensitization in DHR, but are associated with a risk for a severe reaction and may be contraindicated. In vitro tests offer the potential to support or confirm a diagnosis of DHR and influence medical decision making. For immediate-type DHR, a few validated assays for measurement of specific IgE (sIgE) are commercially available to a limited number of drugs...
