BACKGROUND: Laparoscopic cholecystectomy (LC) has become the standard for treatment of uncomplicated cholecystolithiasis. However, in cases of cholecystitis or cholecysto-choledocholithiasis, technical and skill requirements may make questionable the convenience of laparoscopy. Transcylindrical cholecystectomy (TC) is a modified minilaparotomy, performed gas-free through a single cylinder 3.8 cm in diameter and 10.0 cm in length. Our study objective has been to assess the efficacy and potential advantages of TC in the treatment of cholelithiasis, cholecystitis, and cholecholithiasis through an efficacy, prospective, and longitudinal study...

AIM: to determine the safety of simple laparoscopic cholecystectomy in ageing patients. METHOD: the outcome of patients between 60 and 70 years of age and patients over 70 who underwent laparoscopic cholecystectomy for symptomatic non-malignant gallbladder disease was comparatively analysed. All patients over 60 years of age with symptomatic gallbladder disease and without cholecholithiasis, septic shock, diffuse peritonitis, gallbladder malignancy, portal hypertension or contraindication for general anaesthesia were selected for simple laparoscopic cholecystectomy (n = 158)...

Nonsurgical methods compete with common bile duct exploration (CBDE) in the treatment of choledocholithiasis. Following CBDE residual duct stones are found in 6% of cases and the mortality is 2%. In our patients (n = 373) the numbers were 5.4% and 1.6% respectively. The lowest rates of residual duct stones were seen following a combination of intraoperative cholangiography and choledochoscopy. CBDE is indicated when cholecholithiasis is discovered intraoperatively, in low-risk patients with gallbladder and bile duct stones, after unsuccessful endoscopic procedures, in cases, of other pathologic findings of the bile duct requiring surgery or even in younger patients after prior cholecystectomy...
