OBJECTIVE: The purpose of this study was to estimate the cost differences between elective cesarean delivery and the alternative of attempted vaginal delivery and to assess the economic impact of cesarean delivery on demand. STUDY DESIGN: Cost data were obtained over a 12-month period from a not-for-profit community hospital to calculate a per-patient cost for clinical alternatives. RESULTS: The average cost of an attempted vaginal delivery without oxytocin (Pitocin) or epidural anesthesia was 15...

QUESTION: Recently I observed the use of vaccuum aspiration in an attempt to induce abortion in a woman 16 weeks' pregnant, after dialation of the cervix to 14 mm under paracervical block anesthesia. Only amniotic fluid and a segment of the umbilical cord could be aspirated. The patient was returned to bed, and an oxytocin (Pitocin) infusion was begun. She aborted 24 hours later with no complications. Why is this procedure considered inferior to amniocentesis and saline solution instillation as a technique for inducing abortion? ANSWER: Transcervical rupture of the fetal membranes for the purpose of inducing second trimester abortion is unreliable as a method and is fraught with the most serious complication of intrauterine infection (Schwartz, R...

OBJECTIVE: Our objective was to establish whether intrathecal narcotics for obstetric analgesia offer an adequate and cost-effective alternative to epidural analgesia with minimal side effects in our small, semirural community hospital with limited anesthesia coverage. STUDY DESIGN: Low-risk patients at > or = 35 gestational weeks in active labor were offered intrathecal narcotics. A retroactive chart review of every patient receiving an intrathecal injection was compared with a chart review of the next consecutive low-risk patient who did not receive an intrathecal narcotic...

Oxytocin (OT) metabolic clearance rates (MCR) were measured during the constant infusion of 4 mU/min synthetic oxytocin (Pitocin) in 22 healthy adult subjects: 6 men, 6 nonpregnant women, and 10 pregnant women. Pregnant subjects were at term and undergoing OT induction of labor. Mean (+/- SEM) baseline plasma OT concentrations were similar for men, pregnant women, and nonpregnant women. OT MCR was 27 +/- 1.8 ml/kg/min in men; 20.6 +/- 2.8 ml/kf/min in nonpregnant women; and 23.1 +/- 2.6 ml/kg/min in pregnant women...

Six dairy cows were treated before milkings with either oxytocin (Pitocin, 20 i.u.) or ACTH (Synacthen, 150 i.u.), principally to determine their effect on the ratio of citrate: lactoferrin concentrations in the milk. With ACTH treatment, after 3 d milk yield and citrate concentration decreased significantly, lactoferrin and bovine serum albumin (BSA) concentrations increased significantly. Somatic cell counts (SCC) increased temporarily in the milk of three of the cows which previously had greater than 100 000 cells/ml...
