The phenomenon of reverse mismatched ventilation-perfusion on pulmonary scintigraphy is a fairly common occurrence. We present a patient who was experiencing decreasing oxygen saturation and had a reverse mismatched ventilation-perfusion imaging pattern associated with radiotracer retention in a main bronchus.
