Family history as a screening technique has received little attention from epidemiologists as a means of early detection of disorders. In this paper, we present several every-day family cases that will prepare the family physician to make use, at will, of this special investigative tool. We deal primarily with Down's syndrome, defects of the central nervous system, infertility, metabolic disorders and, finally, marriage of close relations. For each case, the role to be played by the physician dealing with a positive family history involving a problem is detailed in terms of risks of occurrence and recurrence of the condition; particular aspects of the counselling process are presented, especially the logistic, psychological, ethical and social aspects...

To develop guidelines regarding prevention and treatment of pelvic organ prolapse and stress urinary incontinence in patients qualified to hysterectomy. The Polish Gynecological Society formed a work group to develop guidelines. The work group formulated the following guidelines: 1. Since the hysterectomy per se is a risk factor for pelvic organ prolapse, more conservative procedures to hysterectomy should be considered and applied, whenever it is possible. 2. Pelvic organ prolapse is not the indication for hysterectomy...

REVIEW

Paradigm shift is moulded by the rapid advances in scientific knowledge, the results of evidence-based medicine and the guidelines and recommendations resulting therefrom as well as the continuous changes in medicine's social-cultural ambient. Features of a complex medical and social development are the declining number of births, a significant increase in the frequency of caesarean sections and an almost two-fold elevation of the number of induced births. This is especially valid for an overproportional rise in elective induction of labour which, today, represents the most frequently indicated intervention in obstetrics...
