Milk thistle has been known for more than 2.000 years as a herbal remedy for a variety of disorders. It has mainly been used to treat liver and gallbladder diseases. Silibum marianum, the Latin term for the plant, and its seeds contain a whole family of natural compounds, called flavonolignans. Silimarin is a dry mixture of these compounds; it is extracted after processing with ethanol, methanol, and acetone. Silimarin contains mainly silibin A, silibin B, taxifolin, isosilibin A, isosilibin B, silichristin A, silidianin, and other compounds in smaller concentrations...

A hepatoprotective effect of thiophan was studied on the model of carbon tetrachloride-induced hepatitis in rats. Therapeutic administration of thiophan repairs the antitoxic function of liver, normalizes cytolysis marker activity, and improves the synthetic function of liver and the carbohydrate and lipid metabolism. The hepatoprotective activity of thiophan is similar to effect of silimarin.

Arsenic (As) compounds are reported as environmental toxicants and human carcinogens. Exposure to arsenic imposes a big health issue worldwide. Silibinin (SB) is a major flavonolignan compound of silimarin and is found in milk thistle of Silybum marianum. Mussaenda 'dona aurora' (sepals) has been investigated for its hepatoprotective and antioxidant activities. The highest activity was observed in the ethyl acetate fraction. The separation of the ethyl acetate fraction gave two iridoids, sanshiside-D and lamalbide. Sanshiside-D exhibited a hepatoprotective activity greater than silimarin as was evidenced by significant reduction of ALT and AST in the serum enzyme levels.

In rats with experimental encephalopathy caused by intoxication with 4-pentenoic acid inhibiting beta-oxidation of medium- and long-chain fatty acids, hepatoprotector silimarin inhibited LPO, prevented deenergization and maintained high respiratory activity of brain mitochondria, and increased the rate and coupling of oxidation and phosphorylation. Succinic acid improved oxidation of substrates in motochondria and promoted activation of succinate-dependent ATP generation. Silimarin and succinic acid used together produced a synergistic protective effect on brain mitochondria surpassing the protective effects of individual preparations and prevented LPO activation...

There is now some evidence that chronic fatigue syndrome is accompanied by an activation of the inflammatory response system and by increased oxidative and nitrosative stress. The influence of silistrong on the activity of erythrocyte cytoplasmatic dehydrogenases and also purified glyceraldehyde-3-phosphate dehydrogenase (GAPH) was studied. Silistrong and its biologically active components (ethanol and silimarine) were found to activate GAPDH, lactate dehydrogenase (LDH), alpha-glycerophosphate dehydrogenase (GPDH) both in lysates of erythrocytes and in isolated GAPDH. The results of experiments on white rats with CCl4-induced hepatitis showed that the combined introduction of cyclophosphamide and hepatoprotectors of the plant origin (lochein, maksar, silimarin) leads to a decrease in acute toxicity and to less pronounced manifestations of hepatotoxicity of cyclophosphamide. Hepatoprotectors decrease the activity of aminotransferases, reduce the level of bilirubin, normalize the content of protein andpipids in the blood serum, and suppress the development of morphological disorders in the liver...

A ten-day course of karsil (silimarin, legalon) or silibor therapy improved the absorption (as indicated by ueviridin test) and oxidative-antitoxic (as shown by antipyrine test) hepatocyte functions in patients with non-insulin-dependent diabetes mellitus with hepatosis. A similar ten-day course of Liv-52 did not change the studied liver parameters. Inducto-thermal exposure of the splenic area (a 20 min session daily for 5 days) improved the absorption and antitoxic functions of the liver in diabetics. A positive effect of splenic inductothermia persisted for at least two weeks...

Total and nonsedimented activities of lysosomal proteinases were studied in rat liver and kidney tissues after administration of silimarine simultaneously with tetramethylthiuram disulfide (TMTD). Silimarine did not affect the pattern of alterations in total activity of lysosomal proteinases, caused by the TMTD treatment, but the nonsedimented activity of the enzymes studied was normalized. Silimarine appears to exhibit the membrane-stabilizing effect.

A double blind study. Antioxidant and antiperoxidative effects of the free radical scavenger agent silymarin (Legalon) were investigated in patients with chronic alcoholic liver disease in a double blind clinical trial. Six month treatment (at a daily dose of 420 mg) with silymarin significantly enhanced the originally low superoxide dismutase activity of erythrocytes and lymphocytes and also restored the diminished superoxide dismutase expression on lymphocytes as measured by flow-cytofluorimetry. Further systematic study of the relation between drugs and the ultrastructure of the liver is reported with regard to the experimental administration of silimarin to pregnant women and others on the pill. Marked signs of ultrastructural alteration of the REL and biliary cell pole were noted, matched, by evidence of throbophilia and changes in protein activity and lipid synthesis, are noted in these situations, but not when suitable doses of silimarine are taken with the pill. It is suggested that silimarin may prevent and correct liver damage during pregnancy and the administration of oestroprogestins...
