Results are submitted of comparative methods of antenatal preparation and delivery in women with varying degree severity of late gestoses, with making use of group E prostaglandin drug preparations that have a marked hypotensive effect. A conclusion has been reached that it is useful to employ prostaglandin drugs (propedil gel, prostin E) in case of advanced labor in women with late gestoses.

Prostaglandins are mainly used in clinical medicine for midterm abortion and to terminate pregnancy. Systemic adverse reactions include nausea and vomiting, which occur in approximately half of the patients and, to a lesser extent, diarrhea. Although bronchospasm occurs infrequently, PGF2 should be avoided in asthmatics. Cardiorespiratory failure culminating in prolonged coma and death has been reported. Moreover, convulsions and EEG changes have been observed in a comparatively small number of cases.

Low dose prostaglandin E2 (PGE2) vaginal gel has been shown to be an effective method for preinduction ripening of the cervix. A method for preparation of this gel from commercially available reagents is described. A twenty milligram PGE2 vaginal suppository ( Prostin , Upjohn) is added to 100 cc's of hydroxyethyl cellulose gel (K-Y Jelly, Johnson and Johnson), heated to 85 degrees C. Samples of this preparation were stored at a 4 degrees Centigrade (C) or 28 degrees C for one, six, or twenty-four hours. We have reviewed a hundred terminations of pregnancy in the second trimester carried out in Geneva, using repeated intra-muscular injections of prostaglandin analogues. We have compared the efficacy and the importance of the side effects of two of these analogues, namely Dinoproston ( Prostin F2 alpha) and Sulproston ( Nalador 500). The induction-delivery interval was identically the same. On the other hand Sulproston provoked fewer cases of diarrhoea (18%) than Dinoproston (69%).

When induction of labour is being considered the first step should be to assess the cervical score. If this is low, the gestational age should be confirmed since an unripe cervix is normal in earlier pregnancy and there may be a mistake in the patient's dates. If the maturity is confirmed, the choice lies between delivering the baby and waiting. Few genuine indications for induction change by waiting and increased risks may occur by postponing the date of delivery. The choice may then lie between elective caesarean section or amniotomy and intravenous oxytocin with an unripe cervix on the one hand, and cervical ripening before induction of labour on the other (Lancet, 1979)...
