Chronic myeloid leukemia (CML) is usually treated with either alpha-interferon or hydrea. Median survival is 6 years. Eventually, in most CML patients, the disease evolves to blast phase with clinical and morphologic characteristics of an acute leukemia. This phase is commonly associated with systemic symptoms and the appearance of new cytogenetic abnormalities. Therapy for this phase is of limited value, resulting in a mean survival of 4 months. Hydroxycarbamide (the brand name: Hydrea) was found effective to chronic myelogenous leukemia (CML) in Japan. In the preclinical study, this compound was active against mouse leukemia L 1210 and inhibited DNA synthesis. Clinically, Hydrea was given orally at the daily dose of 500-2,000 mg, dividing 1-3 times. For the maintenance therapy after remission induction, daily dose of 500-1,000 mg was given, dividing 1-2 times. Massive doses of hydroxyurea (Hydrea) were given to 35 patients suffering from different cancers. Protection to the hematopoietic system was obtained by autogenous marrow transfusions. The clinical response obtained was not considered superior to the results obtained by administering the drug orally over a prolonged period. The described technique is safe and relatively atraumatic, and it may be advisable to administer a large priming dose by this technique to be followed by smaller maintenance doses of a given anticancer chemotherapeutic agent...

This case illustrates the danger of Hydrea taken for more than three years, even though results of monthly blood studies and platelet counts were within normal limits. In this patient, thrombocytopenia suddenly developed only five weeks after normal laboratory results. Fortunately, the sodium level reverted to normal and the pancytopenia disappeared within a week with no residual effects.

The formation of struvite stones is closely linked to the enzyme urease produced by certain bacteria found in urine. It has been shown that hydroxamic acids will block the activity of this enzyme. One of these, hydroxyurea, in a morning dose of 500 mg. was used in 21 patients for three months or longer. Most were then able to acidify their urine despite persistent urinary infection.
