Since the first transvenous pacemaker implantation, that took place 50 years ago, important progress has been achieved in pacing technology. Consequently, today, more than 700,000 pacemakers are implanted annually worldwide. However, conventional pacemakers' implantation carries a non-negligible risk of periprocedural and long-term complications associated with the transvenous leads and pacemaker pocket. We present a case of an unusual fracture and entrapment of the tip of an angioplasty guide wire within the struts of a previously implanted drug-eluting stent. The patient was a 56-year-old man who had already been treated with two drug-eluting stents in the distal right coronary artery and who needed a new coronary angioplasty of the same vessel proximal to the previous implanted stents. Familial hypercholesterolemia commonly manifests with early onset of atherosclerosis, remarkable skin and tendon xanthomas. Cutaneous xanthomas mostly appear in epidermis. The xanthomas of this case appeared in subcutaneous of ear and sacral region with large bulk. The subcutaneous xanthomas grew larger than before after surgical removal.

Takotsubo syndrome has emerged as a unique form of acute myocardial injury associated with chest pain or dyspnea, new ST-T segment abnormalities, serum cardiac enzyme release, distinctive transient regional myocardial dysfunction and absence of significant coronary disease at coronary angiography. The initial diagnosis of Takotsubo syndrome at the emergency room is challenging and although its clinical manifestations usually spontaneously resolve over time, it is not always a disease with benign course. Future prospective studies may be helpful to explore the clinical features and prognostic markers for the outcome of patients diagnosed with Takotsubo syndrome...
