The incidence of Cardiac (Cardio-Pulmonary) Arrest (CA) in the Catheterization Laboratory is not common. However, not only does it happen occasionally but its frequency tends to increase due to the increase in the overall number and complexity of interventions performed by interventional cardiologists. Therefore, certified knowledge and application of the ALS (Advanced Life Support) protocols by trained Catheterization Laboratory staff is imperative in many countries, by local Hospital standards and also by Law...

Constantinos H Papadopoulos, Dimitrios Oikonomidis, Efstathios Lazaris, Petros Nihoyiannopoulos
Cardiac arrhythmias refer to any abnormality or disturbance in the normal activation sequence of the myocardium and may be indicative of structural heart disease as well as the cause of significant cardiovascular complications and sudden cardiac death. The following review summarizes the current state-of-the-art knowledge on the role of echocardiography in the management of cardiac arrhythmias and focuses on atrial fibrillation and ventricular arrhythmias where echocardiography presents a particular diagnostic and prognostic interest...

BACKGROUND: Conventional echocardiographic parameters, such as rest ejection fraction, perform poorly regarding the prediction of exercise tolerance in heart failure. The aim of the present study was to evaluate the contribution of hemodynamic instability in the observed lower functional capacity, investigating the role of left ventricular strain for the prediction of stress test duration in obese hypertensive patients with reduced ejection fraction. METHODS: Sixty-one patients with reduced ejection fraction underwent treadmill exercise echocardiography...

BACKGROUND: Heart failure (HF) is a syndrome that results in inadequate blood supply, leading to a number of structural and functional changes. Noninvasive ventilatory support (NIVS) is used as an adjuvant treatment to improve the functional capacity of these patients. OBJECTIVE: To investigate the effect of NIVS with bilevel positive airway pressure ventilation (BiPAP) on exercise tolerance and dyspnea in HF patients. METHODS: Forty patients with New York Heart Association class I/II/III HF were randomly assigned either to a NIVS group (n=20) or a control group (n=20)...

OBJECTIVES: To research vitamin D levels in the prevention of new-onset atrial fibrillation(AF), we conducting a systematic review of randomised controlled trials (RCTs). METHODS: The MEDLINE, EMBASE and Cochrane Library were searched for English-language RCTs published before September 2016.We focus on the vitamin D levels in the prevention of new-onset AF. The outcomes assessed were vitamin D levels, left ventricular ejection fraction (LVEF) and Left atrium diameter...

BACKGROUND: The aim of our study was to compare the effect of interventricular (VV) delay optimisation in CRT recipients based on systolic dyssynchrony index (SDI) derived from the three-dimensional echocardiography (3DE) versus QRS width assessment, on left ventricle volume reduction at the 12-month follow-up. METHODS: We included 63 patients with recently implanted CRT in this randomized, open-label trial. Patients were randomized to VV delay optimisation based on QRS complex width measurement in the group 1 (n=31) to obtain the narrowest QRS complex and based on SDI in group 2 (n=32) to achieve its lowest possible value...

Dimitrios Tsikas, Manfred Kinzel
BACKGROUND: Circulating inorganic nitrite (NO2(-)) is an abundant nitric oxide (NO) reservoir. Renal carbonic anhydrase (CA) mediates the absorption of urinary nitrite. The urinary nitrate-to-nitrite molar ratio UNOxR is a measure of nitrite-dependent renal CA activity. The CA family is also involved in the bioactivation of nitrite to S-nitrosothiols and NO. Thus, CA isoforms may combat atherosclerosis. METHODS: In plasma and urine of ten hypogonadal men we measured the major NO metabolites nitrite and nitrate, and the endogenous inhibitor of NO synthase (NOS) asymmetric dimethylarginine (ADMA) before and after a 24-weeks testosterone treatment to normalize plasma testosterone levels...

BACKGROUND: The diagnosis of heart failure (HF) remains difficult. A wide variety of studies have indicated that microRNAs play important roles in the pathophysiology of HF. HYPOTHESIS: Circulating miRNAs may be the potential biomarkers for HF. METHODS: In this study, a genome-wide plasma miRNA expression analysis was performed in thirteen HF patients and three controls. Microarray was used for the initial screen, followed by validation with quantitative real-time PCR (qRT-PCR) assays in an independent cohort of thirty-three HF patients and twenty controls...
