BACKGROUND: Owing to mediastinal and cardiac damage burden, the surgical treatment of radiotherapy-related mitral regurgitation (MR) may be associated with high operative risk, or might even be contraindicated. METHODS: /Results: We evaluated the feasibility and outcome of MitraClip therapy in patients with radiotherapy-related MR as an alternative to surgery. Based on Doppler Echocardiography, 15 of 33 screened patients underwent MitraClip implantation leading to MR improvement (residual MR <2+) without significant mitral valve stenosis (planimetric area 2...

BACKGROUND: Cardiovascular disease is the leading cause of mortality in renal transplant recipients (RT). Coronary artery disease (CAD) in such patients is poorly studied. METHODS: Between 2012 and 2017, 50 patients with a functioning renal graft (>6 months) were subjected to coronary angiography in our institution. They were matched (for age, gender, diabetes and indication for angiography) with 50 patients with end-stage renal disease (ESRD) undergoing chronic dialysis and 50 patients with normal renal function, that were subjected to coronary angiography during the same period...

Individuals with hypertension that engage in regular exercise comprise a special patient group that needs a careful approach to differentiate hypertensive cardiac damage from physiologic cardiac adaptations. Echocardiography is the diagnostic modality of choice in such cases. Hypertensive left ventricular hypertrophy is expected to resemble isometric exercise-associated hypertrophy in some but not all cases. On the other side, the hearts of regularly exercising individuals are expected to be normal or present with a variable mix of increased end-diastolic volume and increased wall thickness...

There is a cardiovascular unfavorable duet consisting of arterial hypertension and diabetes that is related with increased risk of heart failure. The latter is highly prevalent in this setting of dysregulation of glucose metabolism and high hemodynamic load and identifying predictors of incidence of heart failure with preserved ejection fraction (HFpEF) is a research and clinical need. Towards this end the variability of glucose regulation represent a novel dynamic way to comprehend and study the impact of glycemia...

Chronic obstructive pulmonary disease is a common respiratory condition that presents in varying degrees of severity and can be complicated by further co-morbidities. Up to a third can also have pulmonary hypertension, which is an important risk factor associated with an increase in morbidity and mortality. The etiology of the pulmonary hypertension contributes. Correctly considering and identifying the type of pulmonary hypertension will influence management and outcome in patients with COPD. The article by Lepida et al on the contribution of left heart disease in chronic obstructive pulmonary disease (COPD) with pulmonary hypertension (PH) investigates and describes a 'real population' clinical prevalence of two known related conditions...
