Familial hypercholesterolemia commonly manifests with early onset of atherosclerosis, remarkable skin and tendon xanthomas. Cutaneous xanthomas mostly appear in epidermis. The xanthomas of this case appeared in subcutaneous of ear and sacral region with large bulk. The subcutaneous xanthomas grew larger than before after surgical removal.

Takotsubo syndrome has emerged as a unique form of acute myocardial injury associated with chest pain or dyspnea, new ST-T segment abnormalities, serum cardiac enzyme release, distinctive transient regional myocardial dysfunction and absence of significant coronary disease at coronary angiography. The initial diagnosis of Takotsubo syndrome at the emergency room is challenging and although its clinical manifestations usually spontaneously resolve over time, it is not always a disease with benign course. Arterial hypertension and aortic valve stenosis are common disorders and frequently present as concomitant diseases, especially in elderly patients. The impact of hypertension on heart haemodynamics is substantial, thus affecting the clinical presentation of any coexisting valvulopathy, especially of aortic stenosis. However, the interaction between these 2 entities is not thoroughly discussed in the European or/and American guidelines on the management of hypertension or/and valvular heart disease.
