An outbreak of Shigella sonnei infection in a long-stay nursing centre was detected during routine surveillance of notifications in July 1999. Subsequent investigations identified 13 cases of multi-resistant S. sonnei infection affecting nine staff, three community members associated with the centre and one resident of the centre. Each isolate of S. sonnei was genetically indistinguishable. The outbreak investigation identified contact with residents with vomiting and diarrhoea as a significant risk factor for infection amongst staff providing nursing care...

A long-term programme of surveillance of antimicrobial resistance in Neisseria gonorrhoeae isolated in the World Health Organization's Western Pacific Region Gonococcal Antimicrobial Surveillance Programme ((WHO WPR GASP) continued in 1999. Over 10,000 gonococci were examined in 18 focal centres. Resistance to the quinolones and penicillins was already high in many parts of the Western Pacific Region and increased further in most centres, the exceptions being a number of Pacific Island States. Vaccine preventable disease notifications for Australia with disease onset dates between January and March 2000 are reviewed. During this quarter, numbers of notifications for Haemophilus influenzae type b disease and measles were the lowest ever recorded, while those for rubella were the lowest recorded since before the epidemic of spring 1992. These are promising trends that are likely to represent a true reduction in disease incidence. Numbers of pertussis notifications declined compared with the last quarter of 1999, but remain high, making up 88% of notifications for the vaccine preventable diseases reported here...

A case-control study was conducted to investigate the cause of a sudden increase in cases of cryptosporidiosis notified to the Brisbane Southside Public Health Unit from January to March 1998. Fifty-two eligible cases were identified over a three-week period early in 1998. Thirty-one of these cases and 21 control subjects participated in the study. Swimming in the 2 weeks before onset of illness was identified as a likely risk factor for cryptosporidiosis infection (OR 3.1, CI 0.8-12.6, P = 0.06). Notification records of typhoid and paratyphoid cases among residents of south-eastern Sydney during 1992-1997 were reviewed, with particular attention paid to identifying a source of infection and to completeness of follow up. Notifications comprised 30 cases of Salmonella Typhi, nine of S. Paratyphi A and five of S. Paratyphi B. These 44 cases had a median age of 20 years (range 2-62). Of the 39 cases with known country of birth, 30 were born overseas, predominantly in Asian countries. We report on three nursing home outbreaks of gastroenteritis in Brisbane in 1999. The presence of Norwalk-like virus (NLV) genogroup 2 was demonstrated by reverse transcription polymerase chain reaction (RT-PCR) in all three outbreaks. Common findings of these investigations were rapid spread of the illness within the institutions and difficulties in identifying a common source. The incidence of hepatitis A virus (HAV) in south-eastern Sydney is one of the highest in Australia with large outbreaks previously associated with male-to-male sexual contact. We report HAV notification trends over the period 1 June 1997 to 31 May 1999 for this location. In all, 18 cases caused by Legionella pneumophila serogroup 1 were notified within 10 days making this the largest outbreak in Victoria reported to that date. The National Rotavirus Reference Centre has conducted rotavirus surveillance by means of a collaborative laboratory based initiative started in June 1999. The serotypes of rotaviruses that lead to the hospitalisation of children with acute diarrhoea were determined from June 1999 to May 2000. We examined 1126 rotavirus specimens using a combination of monoclonal antibody immunoassay, reverse transcription-polymerase chain reaction, and hybridisation.
