BACKGROUND: Apremilast is a "small molecule", an oral phosphodiesterase 4 inhibitor indicated for the twice- daily treatment of adults with psoriasis (PsO) and psoriatic arthritis (PsA). Data concerning clinical experience in real world setting with Apremilast for psoriasis are still exiguous, and aim of this report is to provide our experience in the use of Apremilast in out-patient setting. METHODS: Ten caucasian individuals (6 male, 4 females; mean age 69,3; range 53-81 years) affected by moderate to severe plaque psoriasis (PASI≥10 e/o DLQI≥10 e/O BSA≥10) were treated with Apremilast, following dosing regimen of technical data sheet and clinically evaluated both after 12 weeks (T12) and 16 weeks (T16)...

BACKGROUND: Transplant patients need to be strictly followed, since the immunosuppressive therapies they usually receive can increase the risk of skin complications. This study aims to evaluate the prevalence of neoplastic skin complications in transplant patients. METHODS: We analyzed 256 liver or kidney transplant patients. BACKGROUND: Xerosis and atopic dermatitis (AD ) are chronic skin conditions that occur in children and adults which can result in scaling, flaking and itching. Risk factors for xerosis include sunlight, friction, low humidity and use of soaps. Xerosis is also a symptom of cutaneous conditions such as psoriasis, dermatitis and ichthyosis. AD has a complex pathogenesis but there is increased evidence that a genetically-impaired skin barrier plays a primary role in its development. Ultraviolet B (UVB) irradiation activates aryl hydrocarbon receptor (AHR), generates reactive oxygen species (ROS) and mediates photocarcinogenesis and photoaging. 6-Formylindolo[3,2-b]carbazole (FICZ) is a tryptophan photoproduct generated by UVB exposure. FICZ exhibits similar biological effects to UVB, including AHR ligation and ROS production. FICZ also acts as a potent photosensitizer for UVA and the production of ROS is synergistically augmented in the simultaneous presence of FICZ and UVA. Psoriasis is a chronic inflammatory skin disease with under-investigated and underappreciated links to psychiatric comorbidities, including anxiety, depression, and suicidality.
