Y Li, H Tian, K Zhang
Recurrent hemarthrosis is a relatively rare complication after total knee arthroplasty with the incidence estimated between 0.3% and 1.6%. There are various causes of this complication which can be broadly divided into general and local reasons. If the blood coagulation function was normal, the most common reason of hemarthrosis is the impingement of proliferative synovium between the articulating components of the knee prosthesis. However, some causes are not well understood which make diagnosis and treatment more difficult...

Fluid therapy is an important part of perioperative period, also one of the most controversial issues. Having reviewed the relevant research in recent years as well as the large-scale meta-analysis, the perioperative goal-directed fluid therapy has been discussed from the aspects of evaluating indicators, new methods and latest progress, and the impact on the prognosis. It manifests that the development of goal-directed fluid therapy makes a better prognosis than traditional fluid therapy, therefore it has also became an important perioperative treatment strategy...

Currently surgical resection is still the best therapeutic method to treat hepatocellular carcinoma, but the effects of different surgical methods on postoperative recurrence of hepatocellular carcinoma are still controversial. According to the technical points and developmental course of anatomical and non-anatomical hepatectomy, this paper analyzes the published literature and draws a conclusion that success of hepatectomy depends on exact balance between residual liver reserve function and radical resection of tumor...

The shortage of donor lung remains one of the major problem for lung transplantation. With the development of modern lung preservation and repair technique, increasing marginal lung donors have been re-assessed and finally utilized for transplantation. The ex vivo lung perfusion technique (EVLP) was designed and has been developed for evaluation and repair of the lung. Nowadays, the indication of EVLP and the standard of qualified donor lung have reached a consensus according to the foreign publications. The EVLP system could be classified into three categories: the Toronto technique, the Lund technique and Hannover-Madrid technique...

Bypass surgery(BSX) and endovascular therapy(EVT) are the most important therapeutic method to critical limb ischemia.EVT has potential advantages in reducing surgical trauma and early postoperative complications, shortening hospital stay and so on. Concerning long-term results, BSX is better in reducing long-term mortality and improving long-term patency than EVT group. Therefore, control indications reasonably and select individualized methods, avoid the abuse of EVT are more meaningful for patients.

Despite rapid progress, clinical lung transplantation in China still lags far behind. A great challenge remains in donor lung utilization and perioperative medicine. It's really abnormal that we are so backward in lung transplantation when we have come up with the advanced world levels in thoracic surgery, pulmonology and critical care medicine. Our shortcomings were analyzed by comparing lung transplantation in China and in the advanced countries. The first problem is multidisciplinary teamwork. In the United States, a lung transplant team includes physician specialized in lung transplantation, thoracic surgeons, nurses, respiratory therapists and other specialists possibly needed...
