OBJECTIVES: This study aimed to evaluate the short-/mid-term outcome of patients with complex dextro-/levo-transposition of the great arteries (d-/l-TGA), ventricular septal defect (VSD) and left ventricular outflow tract obstructions (LVOTO). METHODS: A single-center, retrospective review of all complex d-TGA (n = 85) and I-TGA (n = 22) patients undergoing different surgeries (Arterial switch operation (ASO)+LVOTO-resection (ASO-R), half-turned truncal switch/Mair (HTTS), Nikaidoh and Rastelli) between 05/1990-09/2022 was performed...

OBJECTIVES: To review our surgical experiences in patients with complex pathology of the aortic arch who have undergone antero-lateral thoracotomy with partial sternotomy (ALPS). METHODS: From October 2019 to November 2023, 23 patients underwent one-stage repair of complex pathology of the aortic arch through the ALPS approach. The mean age was 61.9 ± 16.7 years old. The aortic pathology was as follows: aorta-related infection in eleven (aorto-oesophageal fistula: four, graft infection: six, native aortic infection: one), aortic dissection in nine including shaggy aorta in two, non-dissecting aneurysm in one, and coarctation of the aorta (CoA) in two...

OBJECTIVES: Intestinal ischemia-reperfusion injury induced by cardiopulmonary bypass causes intestinal epithelial barrier dysfunction, leading to dysbiosis and bacterial translocation. We conducted a randomized prospective study with two objectives: (1) to investigate epithelial barrier dysfunction and bacterial translocation induced by cardiopulmonary bypass and changes in the gut microbiota and (2) to verify whether probiotics can improve these conditions. METHODS: Between 2019 and 2020, patients 0-15 years old scheduled to undergo cardiac surgery using cardiopulmonary bypass were enrolled and randomly allocated to 2 groups: the intervention group received probiotics, and the control group did not receive probiotics...

BACKGROUND AND OBJECTIVE: The Haller index (HI) is widely utilized as a quantitative indicator to assess the extent of pectus excavatum (PE) deformity, which is the most common chest wall abnormality in children. Both preoperative correction planning and postoperative follow-up need to be based on the standard of normal thoracic growth and development. However, there is currently no established reference range for the HI in children. Consequently, the aim of this study was to conduct a preliminary investigation of normal HI values among children to understand thoracic developmental characteristics...

OBJECTIVES: To assess the current practice of pulmonary metastasectomy at 15 European Centres. Short- and long-term outcomes were analysed. METHODS: Retrospective analysis on patients ≥18 years, who underwent curative-intent pulmonary metastasectomy (01/2010-12/2018). Data was collected on a purpose-built database (REDCap). Exclusion criteria were: previous lung/extra-pulmonary metastasectomy, pneumonectomy, non-curative intent, and evidence of extrapulmonary recurrence at the time of lung surgery...

BACKGROUND: Robotic assisted thoracoscopic surgery facilitates complex pulmonary segmentectomy which offers one-stage diagnostic and therapeutic management of small pulmonary nodules. We aimed to explore the potential advantages of a faster, simplified pathway and earlier diagnosis against the disadvantages of unnecessary morbidity in benign cases. METHODS: In an observational study, patients with small, solitary pulmonary nodules deemed suspicious of malignancy by a multidisciplinary team were offered surgery without a pre or intra operative biopsy...

OBJECTIVES: To assess the early outcomes and risk factors of paediatric patients requiring extracorporeal membrane oxygenation after cardiac surgery (post cardiotomy). METHODS: Retrospective binational cohort study from the Australia and New Zealand Congenital Outcomes Registry for Surgery database. All patients younger than 18 years of age who underwent a paediatric cardiac surgical procedure from 1 January 2013 to 31 December 2021 and required post-cardiotomy extracorporeal membrane oxygenation in the same hospital admission were included in the study...

BACKGROUND: To access early and midterm outcomes of a gutter-plugging chimney stent-graft for treatment of Stanford type B aortic dissections (TBAD) in the clinical trial of Prospective Study for Aortic Arch Therapy with stENt-graft for Chimney technology (PATENCY). METHODS: Between October 2018 and March 2022, patients with TBAD were treated with the LonguetteTM chimney stent-graft in 26 vascular centers. The efficiency and the incidence of adverse event over 12 months were investigated...

Individual patient data (IPD) meta-analyses build upon traditional (aggregate data) meta-analyses by collecting IPD from the individual studies rather than using aggregated summary data. Although both traditional and IPD meta-analyses produce a summary effect estimate, IPD meta-analyses allow for the analysis of data to be performed as a single dataset. This allows for standardization of exposure, outcomes, and analytic methods across individual studies. IPD meta-analyses also allow the utilization of statistical methods typically used in cohort studies, such as multivariable regression, survival analysis, propensity score matching, uniform subgroup and sensitivity analyses, better management of missing data, and incorporation of unpublished data...

OBJECTIVES: Serratus anterior plane block (SAPB) and paravertebral block (PVB) are well known to reduce pain levels after video-assisted thoracic surgery (VATS). However, the relative efficacies of each block and a combination of the two have not been fully characterized. The objective of the present study was to assess the efficacy of PVB alone, SAPB alone, and the combination of PVB and SAPB with regard to the occurrence and intensity of pain after VATS. METHODS: We conducted the THORACOSOPIC single-centre, double-blind, randomized trial in adult patients due to undergo elective VATS lung resection...
